PrefaceRadiation therapy is one of the major treatments of non - small cell lung cancer. The occurrence, development and prognosis have a close correlation with the activation of the oncogene and the inactiva-tion of the anti - oncogene. It's approved by the animal experiments that we can improve the prognosis of the patients undergone radiotherapy by transferring the expression of gene. Radiation resistance and sensibility can be co - induction by polygene, which can influence the results of the radiotherapy.There is a rather high expression of p53 and Bel - 2 gene in lung cancer. The purpose of this experiment is to study the expression of p53, Bel - 2 in the non - small cell lung cancer and the relationship between the expression and the prognosis.Materials and Methods1. MaterialsTo collect the clinical data of non - small cell lung cancer (NSCLC) which undergone radiation therapy in Oncology TreatmentCenter, China Medical University. All the 58 patients have a his-topathologically confirmed diagnosis obtained by bronchoscopy. Sections were 10% formalin - fixed, paraffin - embedded, five - jxm sections were cut from paraffin blocks , stained with hematoxylin and eo-sin conventionally. All patients were treated with Siemens linear accelerator. 15MV X-ray ,Dt = 66 -68GY/33 -34f/45 -49d. SSD = 100cm,and dosage rate were 2Gy/min.2. MethodsStreptavidin - peroxidase ( S - P) immunohistochemistry (IHC) method was used to detect the p53 protein and Bel - 2 protein in primary non - small cell lung cancer tissuesImmunohistochemistry stsining was evalued by two pathology doctors randomly and double - blind. The immunohistochemistry stained sections were judged positive when brown particles appear in nucleus for p53, and in cell plasm for Bel -2.SPSS 10.0 software was used to produce Kaplan - Meier survival analysis, draw survival curve. And log - rank test was applied to determine if there is a relationship between the expression of p53 and Bel - 2 and the prognosis. Using Cox - Regression , make a further confirmation if the expression of p53 and Bel - 2 are independt prognosis factors .Results1. The p53 positive production exists in nucleus,form a serious brown particles. Of 58 cases non - small cell lung cancer patients, There are 32 cases with a positive expression. The positive expression rate was 55.17%.2. The Bel - 2 positive production exists in cell plasm , form a serious brown particles. Of 58 cases non - small cell lung cancer patients , There are 27 cases with a positive expression. The positive expression rate was 46.55%.3. The average survival term of p53 positive expression was 14. 36 0. 97 ( month) , The average survival term of p53 negative expression was 18. 60 1.49 ( month).There is a magnificent diverse relationship between the positive expression of p53 protein with the prognosis of the patients of non -small cell lung cancer(p =0.030).4. The average survival term of Bel - 2 positive expression was 14.39 1.18 (month) , The average survival term of Bel -2 negative expression was 17. 89 1.26 ( month).There is a magnificent diverse relationship between the positive expression of Bel - 2 protein with the prognosis of the patients of non - small cell lung cancer ( p =0.041).5. Age( p = 0. 328 ) and sex( p = 0.588 ) have no relationship with the prognosis of non - small cell lung cancer undergone radiotherapy.6. The results of Cox - Regression: The positive expression of p53 and Bel - 2 are independentprognosis of non - small cell lung cancer undergone radiation therapy( p =0.019).DiscussionThe results of non - small cell lung cancer patients underwent radiation therapy have a very great individual difference. Along with the theory and technique of molecular biology applying in medical research, It's confirmed that apoptosis induced by irradiation depend onp53 protein,and was adjusted by the level of Bcl -2 protein.
