Background:Osteoarthritis of knee(KOA) is a chronic disease which badly harms health of the middle and old age people. KOA is the same meaning as "gw 6/\ knee pain"m traditional Chinese medicine. Its characteristic is that articular cartilage appears primary or secondary degeneration, with hyperplasia of the bone under cartilage. The main clinical expressions of KOA are knee joint pain, stiffness and limitation of activity. The cause and pathogenesis of KOA are not completely clear yet, there is no satisfying way to treat KOA besides replacement of knee joint for few patients in the present medicine. However there were many clinical reports had proved that some traditional Chinese medicine (TCM) had a good effect. My tutor Prof.Xu Shuliang pointed out that KOA is "primary symptoms asthenia and second symptoms sthenia", "primary symptoms wei and second symptoms ?".KOAs that have clinical symptoms were "primary symptoms even and second symptoms acute",and intreatment the primary symptoms and second symptoms shoud be relieved at the same time,the first important is to relieve the second symptoms,then take an eye on primary symptoms.The prescription of gu yan was used to treat KOA by tutor for more than 20 years.But the clinical effect has not been systemly sumed up by now.Objectives:Based on the study about KOA which have been finished by traditional Chinese medicine and Western medicine, experimental study including primary preparation technology, general pharmacodynamic research and clinical investigation was proceeded, in order to lay a foundation for the exploiting and applying of the prescription ofguyan and to exalt the tutor's academic viewpoint.Methods rFirst, review documents relating to KOA and learn its general survey and research progress .Second, lay down technical process of preparation of the prescription of gu yan according to my tutor's clinical experiences , and carry out the acute toxicity experiment and pharmacodynamic study including anti-inflammatory, antioncotic and analgesic.Finally, clinical effect of the the prescription of gu yan was observed comparatively at random, including both the pain of active action and the limitation of arthritis activity in knee.Results:Document review revealed that KOA has been studied deeply by traditional Chinese medicine and Western medicine so far.However, some breakthroughs, such as the relationship between osteoarthritis and osteoporosis, the principle of traditional Chinesemedicine(TCM) to treat KOA and how to embody the doctrine of paying equal attention to bones and muscles and conjuction of movement and fixation by traditional Chinese medicine , are looking forward to be developed. Preparation of the prescription of gu yan was divided into drug granule and capsule by the primary study of preparation. Nux vomica was made .into capsule according to 0.33g per granulation and the other was ^made into drug granule according to 15g per bag. Acute toxicity experiment indicated that clinical daily dosage 2 bags and 2 granulation of the preparation is secure. Pharmacodynamic study confirmed that the prescription of gu yan can reduce the increment of blood capillary permeability caused byhistamine, and can alleviate swelling of auricle and ankle caused by schedul of croton and schedul of egg and can alleviate the pain of mice induced by acetic acid.Clinical observation also show the prescription of gu yan can obviously alleviate the patient's pain of active and passive activity, and its ensemble is better than zhuangguguanjiewan, P<0.01.Conclusions:The prescription of gu yan has good effect on anti-inflammation, detumescence and alleviating pain.The prescription of gu yan is effective on treating KOA, and is worth of further study.
