The iodine deficiency could result in hypothyroidism as well as mental and physical disabilities . Universal salt iodization(USI) is the best solution deprive of the iodine deficiency.At the same time,higher prevalence of hyperthyroidism was reported. It is very significance to supervising the occurrence of hyperthyroidism.The purpose of this study was to investigate the iodine nutrition and the epidemiologic state of hyperthyroidism in ChongQing after universal salt iodization(USI) .Part oneObjective: To evaluate iodine nutrition status in ChongQing after the program of iodized salt supplementation by determining and analysing the urinary iodine of healthy subjects and patients with thyrocele and hyperthyroidism .Methods: 141 healthy subjects and 144 patients with thyrocele and hyperthyroidism were randomly choosed from the outpatient department of the First Affiliated Hospital of ChongQing University of Medical Sciences and ChongQing University.They were respectively 重庆医科大学硕士研究生学位论文 investegated about the history of thyroid disorders.All subjects who were taken as samples needed to accept physical examination. Their sera were measured for TT3 ,TT4, TSH .The urinary iodine was examined by spectrophotometer.Results: The median value of urinary iodine after iodine supplementation were 247μg/L and 224μg/L in normal subjects and patients with thyrocele and hyperthyroidism respectively,which were higher than those before iodine supplementation.While a slight but insignificant difference between normal subjects and patients were found(p>0.05).The value of urinary iodine in 47% of normal subjects and 17% of patients with thyrocele and hyperthyroidism were between 100μg/L and 200μg/L. The value of urinary iodine in 53% of patients with thyrocele and hyperthyroidism were over 200μg/L and 33% of patients were less than 100μg/L.Conclusions: The iodine deficiency was corrected by the supplementation of iodized salt,however ,higher and lower intake of iodine all existed among the patients with thyrocele and hyperthyroidism .Part twoObjective: To investigate the epidemiologic state of hyperthyroidism in ChongQing after universal salt iodization(USI) since 1996 as well as the influence of USI on the incidence of hyperthyroidism. Methods: The incidence of hyperthyroidism were contrasted in pre-and-post supplement 重庆医科大学硕士研究生学位论文 of iodine.In addition to the history , physical examination,thyroid function,thyroid autoantibodies and thyroid B ultrasound were measured for all the subjects.The urinary iodine concentration were determined by spectrophotometer.Results: It showed that the urinary iodine concentration of the population increased from 53.14μg/L to 247μg/L since the iodine nutrition of population had been improved.Most of the case's age were over 40 years old.The number of female was more than that of male in case.There were no difference between the hyperthyroid patients and the general population in content of urine iodine.It indicated even the iodine was supplied properly,the hyperthyroidism still would be attacked among some high-risk population. Conclusion: It suggested that supervising the occurrence of hyperthyroidism should be carried out when supplying iodine to all population in iodine deficiency area.So that we could scientifically control IDD.Conclusion:1. At present,higher and lower intake of iodine all existed among the patients with thyrocele and hyperthyroidism in ChongQing. 2. The iodine nutrition of population had been 重庆医科大学硕士研究生学位论文 improved and higher prevalence of hyperthyroidism was found in ChongQing after universal salt iodization(USI).3. Supervising the occurrence of hyperthyroidism should be carried out among some high-risk population when supplying iodine to all population in iodine deficiency area.
