Objective: Researching the mechanism of Shuizhitang that it increases the amount of microvascular and promote skin flaps to heal.Methods: 100 wister rats are divided into 5 groups at random. The random pattern skin flap model is made on the rat back, which size is 7.5x3 cm 2. Before operation, applying physiological saline, heparin, single leech, VEGF protein, Shuizhitang to treat these rats separately. The survival rate of skin flaps and the expression of VEGF in flap tissues was determined by pathologic histology, RT-PCR, ABC immunohistochemistry methods after operation 1,4,7,14,21 day and before operation.Results: Observation of pathology: Flaps at the far part are swell postoperatively. Then, the swell is alleviative in the flaps of single leech group, VEGF group, Shuizhitang group. The survival rate of flaps: All 5 groups are similar at 4th day postoperatively. The single leech group, VEGF protein group, Shuizhitang group are higher than other 2 groups at 7th day postoperatively. VEGFmRNA: The expression of VEGFrnRNA in all 5 groups is similar at 1st day postoperatively but higher than that in fore-operative tissues.The expression in single leech group, Shuizhitang group is higher than that of the other 3 groups at 4th-7th day postoperative, and the expression of these 2 groups still appears at 14th day postoperatively, but at that time, the expression of the other 3 groups is similar to that of fore-operative. VEGF protein: The expression of VEGF protein in all five groups is higher than that of fore-operative group at 1st day postoperatively, but the expression in VEGF group is the highest. The expression in physiological saline group, heparin group is similar to that of the fore-operative group. The expression in the other 3 groups is rising steadily at that time, and it reaches the peak at the 7th day postoperatively. From then on, it is gradually descended.Conclusion: The expression of VEGFmRNA and VEGF protein is significant increased with vascular growth after having been treated with single leech and Shuizhitang. It appears that the Shuizhitang maybe increase the expression of VEGFmRNA and VEGF protein, and consequently mediate the microvascular growth. Through the kinds of mechanism, Shuizhitang promotes the skin flap to heal.
