Objective:To study the correlation between CT appearance, pathology and PCNA expression in peripheral lung adenocarcinoma, and to search the value of some CT findings in evaluating the biological behavior of peripheral lung adenocarcinoma. Materials and Methods:52 cases of peripheral lung adenocarcinoma confirmed by operation and pathology were examined with CT before operation.The expression of PCNA was assessed in parraffin-embedded tumor samples with the SABC immunohistochemistry stain method, and the correlation between the CT appearance and the overexpresion of PCNA was analysed.Results: ?PCNA expression of tumor with poor differentiation and lymph node metastasis was significantly higher than those with good differentiation and no lymph node metastasis. (2) Peripheral lung adenocarcinoma with CT feature of diameter over 3cm ,deep lobulation , spiculate protuberance and vessel convergency had higher PCNA expression, whereas those with air bronchogram had lower PCNA expression. There was no difference of PCNA expression between the tumors with spiculation or pleura! indentation and those without spiculation or pleura! indentation. (3) The expression of PCNA was higher in peripheral lung adenocarcinoma with obvious contrast enhancement. Conclusion: PCNA expression is correlated with the degree of tumor celldifferentiation and lymph node metastasis.The results indicatethat the expression of PCNA could be an effective criterion to evaluate the proliferation status, the degree of malignancy and metastasis .potential in peripheral lung adenocarcinoma.The peripheral lung adenocarcinoma with diameter over 3cm , deep lobulation, spiculate protuberance, vessel convergency and obvious contrast enhancement might have higher tumor cell proliferation and degree of maglignancy, whereas those with air bronchogram might have relatively benign biological behavior. The peripheral lung adenocarcinoma with spiculation or pleural indentation cann't demonstrate its biological behavior.
