Objective To investigate the expression of vascular endothelial growth factor (VEGF), its receptor KDR and their relationship to metastasis and prognosis in pulmonary carcinoma. Methods The expression of VEGF and KDR was examined immunohistochemistrically by SABC method in 75 patients with pulmonary carcinoma.Results There was an extensive expression of VEGF and KDR, mainly in the cytoplasm of tumor cells (TCs), fibroblasts (FBs), endothelial cells (ECs), and inflammatory cells. Also could be found sometimes in nuclei of small cell lung carcinoma (SCLC). The distribution of VEGF and KDR was identical, mainly located at periphery of the tumor mass or necrosis. The positive rate of VEGF and KDR in the TCs was 61.33%, 46.67% and 53.33%, 48% in the FBs. No significance was found between the expression of VEGF or KDR and age groups, sex groups and pathological type groups in the TCs and FBs (P>0.05). The positive rate of VEGF and KDR in TCs and FBs, in D > 5cm group, 3cm < D 5cm group and D 3cm group, respectively, was 90.32%, 78.57%, 80.65%, 74.19%, 55%, 49%, 45%, 45%, and 29%, 20.83%, 25%, 16.67%. It was obviously different thatthe positive rate of VEGE and KDR among the three groups (P<0.005). The larger the tumor mass was, the higher the positive rate of them was. The positive rate of VEGE and KDR in lung carcinoma with lymph node metastasis was (in TCs was 91.18%, 73.53%, and in FBs was 83.53%, 73.53%.) significantly higher than that in group without lymph node metastasis (in TCs was 36.59%, 38.46%, and in FBs was 26.83%, 26.83%) (P<0.005). In the group of postoperative survival time less than one year, the positive rate of VEGF and KDR was 88.8%, 88.89% in TCs, and was 92.6%, 81.48% in FBs. In the group of 1 to 5 years, the positive rate of VEGF and KDR was 64%, 32% in TCs, and was 52%, 48% in FBs. In the group of more than five years, the positive rate of VEGF and KDR was relatively lower, was 8.7%, 13.04% in TCs and 8.69%, 8.7% in FBs. There was significant relation among the three groups (P<0.005). The higher the positive rate of VEGF and KDR was, the longer the postoperative survival time was. Conclusion Our data suggested that VEGF and KDR play important roles in tumor development, metastasis and pronosis of pulmonary carcinoma.
