| ObjectiverTo report 20 cases of traumatic carotid cavernous fistula,which treated by balloon occlusion via intravascular.We summarized clinical material,methods,results,postoperative complications and therapy. Clinical materialrOf the 20 patients, 16 were male and 4 female, with a mean age of 32.5 years. All patients had chemosis, audible bruit and exophthahnos, of which 16 had restriction of eye movements. DSA showed that 9 patients had the fistula in Ca segment, 5 in Cs.4 segments, 6 in C45 segments, that 13patients drainaged to superior ophthalmic vein,7 to superior and inferior petrosal sinuses,2 to pial cerebral veins. Methods: The femoral artery approach and the technique of detachable balloon catheter were used in all patients. Results: First complete occlusion of fistula was achieved in 18 patients , of whom 16 had ICA preserved. The preservation rate of the flow of ICA was 80%(16/20). The second occlusion was used in 1 patient. No serious complications occurred. Conclusion: The technique of detachable balloon catheter is the best for the treatment of TCCF, in which a few patients may have some complications, but the complications will be avoided if we take some preventative measures.Graduatestudent: Tinglin sui (neurosurgery) Derected by Qinghai meng... |
