Application And Comparison Of Detachable Balloon And Stent-graft In The Treatment Of Carotid-cavernous Fistula

Objective:Carotid cavernous fistula(CCF) refers to the cavernous segment of internal carotid artery or the branch of the cavernous segment of internal carotid artery rupture, whi ch cause the abnormal communication between the artery and the vein. That can ca use the pulsating exophthalmos, intracranial murmur, chemosis or fatal epistaxis etc.At present, the main method for treatment is operation and interventional therapy,Interv entional therapy has become the preferred method for the treatment of CCF. The mai n materials for interventional therapy include detachable balloon, coil, stent-graft etc.Domestic and foreign scholars do more study on the detachable balloon in the treat ment of CCF, but less on the stent-graft. Analysis of detachable balloon and stent-g raft in the treatment of CCF, can contribute to the rational selection of treatment pla n, a clear indication of operation.To clear the advantages and disadvantages in the treatment of carotid cavernous fi stula by detachable balloon and stent-graft, for selecting suitable therapy for clinical re ference.Methods:A retrospective analysis on the cases between the year 2010 to year 2013.A total of 22 cases that 9 patients were treated by detachable balloon in the second hospi tal of Shanxi medical university,8 were treated by detachable balloon in the gener al hospital of pla, and the other 5 were treated by the stent-graft in the same place.Results:The cases treated by the detachable balloon therapy smoothly. The intracranial m urmur ease Immediately after the therapy. Angiography prompt that the fistula was c losed. Only one patient were found by postoperative examination 3 months after op eration, but the compensatory blood flow is good, the patient without any symptoms.The cases treated with stent-graft implantation of stent all success, follow up angiograp hy fistula closure, no leakage, 3 months after operation without recurrence, the homon ymy internal carotid artery patency, no stenosis.Through the statistics of clinical sympto ms to improve the situation analysis, P<0.05, two kinds of treatment methods are effe ctive.Conclusion:For Carotid cavernous fistula patients, the detachable balloon embolization treatme nt is still the main means, but as the technology matures, stent-graft become an importa nt and effective means in interventional treatment of carotid cavernous fistula.Differe nt embolism materials suitable for different situations of internal carotid artery caver nous sinus, and in some cases can also be treated with combined application of var ious embolic materials.
