Objective: graft versus host disease(GVHD) or graft versus host reaction(GVHR) is one of the most common complications after allogeneic hematopoietic stem cell transplantation. GVHD plays an very important role in the successful transplantation and it is a key factor for the clinical prognosis. The GVHD incidence rate is up to 40% to 85% even in the HLA -matched allo-transplantation and it has a strong effect on the prognosis of transplantation patients. The overexpression of cytokines is one of the most important fators in the pathogenesis of GVHD.Methods: In this study we dynamically determined was to exam blood cytokines' level(including IL-2, IL-4, IL-6, IL-8, IL-10, VEGF and IL-l) in 16 malignant patients of 301 and 307 hospital underwent allo-transplantation.Results:(1)IL-2 reached its peak two weeks after transplantation in GVHD group and was higher than that of before transplantation and nonGVHD group (P=0.051) .This peak appeared earlier than GVHD clinical signs did. and reduced gradually with the GVHD signs losing;(2)The level of IL-4 increased rapidly one week after transplantation and decreased thereafter, while the IL-4 concentration in nonGVHD group stayed in low level before and after transplantation, the difference between the two group is significant (P<0.01); (3)Plasma IL-6 level increased rapidly after transplantation in GVHD group and reached its peak 1 week after allo hematopoietic transplantation (p<0.05). In nonGVHD group plasma IL-6 level decreased rapidly after transplantationand hold in low level for a long time;甀L-8 level was much higher in GVHD group than that of nonGVHD group before transplantation . IL-8 blood level rose and reached its peak one week after transplantation (PO.01) ;(?IL-10in nonGVHD group was higher than that of GVHD group before transplantation, this showed that high level IL-10 before transplantation may be benefit for the prevention of GVHD;(6)This study showed that IL-1 a and VEGF level had little difference between GVHD and nonGVHD group before and after transplantation (P>0.05) .Conclusion: the level of IL, IL-4, IL-6which increased rapidly after allo- transplantation indicated aGVHD would occur; The high level of IL-8 befor allo- transplantation indicated the high risk of GVHD;Contrary to IL-8, The high level of IL-10 befor allo-transplantation indicated the low risk of GVHD; This study showed that IL-1 a and VEGF level had little relationship with GVHD . The results from our observation showed that it may have a wide prospect in the future by dynamically observed the level of IL-2, IL-4, IL-6, IL-8, IL-10.
