The secondary lip-nasal deformities present inevitably in the vast majority of cleft lip patients, due to the inherent deficiency, the option of the time and method of the first correction, and the surgery skill. The typical secondary deformities consist of the Cupid's bow disappeared, the vermilion deficit, the deformity of the nasal tip, columella shortened and deviated, the nose alar dome sunk on the cleft side, the lateral crus displaced laterally, asymmetrical nostrils, and so on. The manifestation of the secondary deformities is so varied that the corresponding repairing is complex. We combined several experienced methods and modified them into a convement comprehensive method. We reconstructed the Cupid's bow with the frill- thickness skin excision and vermillion advancement, and repaired the vermilion deficit with the mucosal flap of the lower lip. We corrected synthetically the nosal deformities with the rhombic skin excision on the nostril base, the traction of the columella base and the lateral alar ems, M+T incision on the edge of naris, and expanded polytetrafluoroethylene (ePTFE) implantation. 12 cases were treated with this comprehensive method. The lip- nasal deformities got satisfactorily corrected. No complication was present, and the half-year following manifested the good long-term effect. This comprehensive method can be used to correct satisfactorily the medium or severe secondary lip-nasal deformities in the cleft lip patients over the age of 15.
