Purpose:This article explored that the relation between different syndrome of TCM and blood fat,the relation between coagulation function and color Doppler ultrasound by observing laboratory indexes of patients. It offered modern objective indicator for Chinese tradition medicine type of diabetic foot patients. It could recover deficiency of TCM syndrome differentiation.Material and method : This study is by selecting 33 patients met the inclusion criteria of patients with diabetic foot,syndrome differentiation of them, divided into five syndromes: cold dampness resistance type winding, blood stasis resistance type, deficiency of qi and yin, the dampness-heat toxic pattern,hot poison injury yin, hot poison injury yin, thermal resistance rheumatism flashover poison yin.The patients that were hospitalized nondiabetic(excluding those with diabetes, hypertension and gangrene)were by selecting 33 cases as the control group. We registers basic information of the patients after admission, the first biochemical examination indicators including blood lipid, four items of blood coagulation and lower limb artery color Doppler ultrasound. Statistics with SPSS17.0 software, count data using X2 test, measurement data using t test, multiple sets of mean comparison using ANOVA analysis and LSD test, and carries on the preliminary analysis basis of the correlation.Results:1.In the collection of 33 cases, the patients who got the dampness heat toxictype with the greatest number, account about 51%, the other four groups of about12%. Diabetic foot group and non-diabetic foot group no gender differences between the two groups.(P=0.403>0.05).2.In terms of blood fat the diabetic foot group of low high-density lipoprotein(HDL-C),compared with non-diabetic group was statistically significant(P< 0.05),lipoprotein(a)[LP(a)]is higher than that non-diabetic group, with statistical significance(P<0.05).The type of cold dampness got higher triglycerides(TG),compared with other groups have statistical significance(P<0.05).And the triglyceride(TG) of dampness heat poison is low, with qi and Yin deficiency,blood stasis resistance and cold dampness three groups was statistically difference(P<0.05).Blood stasis resistance type of lipoprotein(a) [LP(a)]is higher than other groups, with statistical significance(P<0.05).Hot poison Yin type of lipoprotein(a) [LP(a)] is higher than that of qi and Yin deficiency,hot and humid poison, was statistically significant(P<0.05).Cold dampness type of lipoprotein(a)[LP(a)]is low, less than the other four groups was statistically significant(P<0.05).Total cholesterol(CHOL),low density lipoprotein(LDL-C)and high-density lipoprotein(HDL-C)little differences between groups, each group has no statistical significance(P>0.05).3.Diabetic foot in the coagulation of four groups of prothrombin time(PT)and activated partial blood coagulation time(APTT) live enzymes to shorten, compared with non-diabetic group was statistically significant(P<0.05),fibrinogen(FIB)and thrombin time(TT), with statistical significance(P< 0.05).Each type of hot poison Yin group of prothrombin time(PT) is higher than the hot and humid poison sheng group and cold dampness group, comparing with statistical significance(P< 0.05).Part activated clotting enzyme live time(APTT) and thrombin time(TT)are higher than other group, with statistical significance(P<0.05).Hot and humid poison group part activated clotting time live enzymes(the APPT) is a hot poison Yin group is low, but higher than that of qi and Yin deficiency and blood stasis resistance group, with statistical significance(P<0.05).Blood stasisresistance groups of fibrinogen(FIB) is higher, compared with the other four groups have statistical significance(P<0.05).4.Hot and humid poison of type pretibial inside diameter, posterior tibial artery diameter and the width of the dorsalis pedis artery diameter were higher than other groups, the inner diameter, pretibial arteries dorsalis pedis artery diameter and qi and Yin deficiency and blood stasis resistance comparison had statistical significance(P<0.05);The posterior tibial artery diameter compared with cold dampness type statistical significance(P<0.05).Conclusion:1. The occurrence of diabetic foot is closely related to the abnormal lipid metabolism, and different syndrome types of the characteristics of diabetic foot have alone lipid metabolism disorders. Diabetic foot group of high-density lipoprotein(HDL-C) less than non-diabetic group. And lipoprotein(a) [LP(a)]is higher than in non-diabetic group. Cold dampness type with triglyceride(TG)for high characteristics, blood stasis resistance and hot poison Yin in lipoprotein(a) [LP(a)] increased.2.The occurrence of diabetic foot and the coagulation mechanism is closely related to different syndrome types of diabetic foot clotting mechanism is different. Diabetes foot prothrombin time(APTT) and activated partial thrombin time(PT) were compared with non-diabetic shorten; Elevated fibrinogen(FIB)and thrombin time(TT) compared with non-diabetic group are extended. Hot poison Yin type on prothrombin time(PT), activated partial clotting enzyme live time(APTT) and thrombin time(TT) to characteristics of dampness heat poison is filled with activated clotting time live enzymes to prolong for characteristics of blood stasis type resistance increased with fibrinogen.3. The occurrence of diabetic foot has close relationship with narrow arteries diameter, different syndrome types of diabetic foot differences between inner diameter changes of the blood vessels. Diabetic foot group pretibial artery diameter of inner and dorsalis pedis artery, posterior tibial artery diameterless than normal. Hot and humid poison artery diameter wide, qi and Yin deficiency and blood stasis resistance type is relatively narrow, two groups may be associated with a longer duration, blood stasis type resistance and may be due to the high fibrinogen(FIB) caused by blood clots too much increased arterial diameter of the stenosis.
