Objective:To retrospectively analyze the curative effect of percutaneous transforaminal endoscopic discectomy (PTED) and discectomy fenestration on treating lumbar disc herniation during1year after operation, compare the advantages and disadvantages of therapeutic effect of PTED and traditional operation or other minimally invasive surgery on treating lumbar disc herniation and its complication and know about relevant factors of PTED to master PTED better, also provide a reference on treating lumbar disc herniation for more orthopedic and spine surgeons.Methods:46cases were retrospectively analyzed,23of them were treated with PTED, others with discectomy fenestration. The data of two sets before surgery, during surgery and after surgery were recorded,,including gender, age, disease duration, segmental lesions, lesion type, removed intercalated disc, complications, etc. Two groups of patients were assessed with VAS and ODI before surgery,6months after surgery,12months after surgery respectively. The surgery efficacy of two sets can be evaluated with the spine surgery group low back pain surgery scoring criteria which is formulated by Chinese Academy of Orthopaedic spine group. The results above all were statistically analyzed. Results:All patients were followed up for more than1year, the curative effect of PTED and discectomy fenestration were compared. The excellent rate of PTED group is91.30%, with86.95%in fenestration group. The excellent rate of PTED group is better than the fenestration group. According to the statistical analysis, P>0.05, there is no statistical difference between the two groups. There is statistical difference between the two groups in amount of removed bite bone, but not in removed intercalated disc..The score of VAS and ODI of6and12months after surgery was better than the preoperative score in both groups, of which difference was statistically significant. But the difference of scores between the two groups had no statistically significance. The difference in the VAS score of6and12months after surgery had no statistically significance, but ODI score did. Conclusion:The curative effect of PTED is good enough as fenestration discectomy, moreover, PTED can achieve the goal of minimally invasive with less bleeding, low cost, small trauma and good recovery which leads to patients’return to society earlier. So PTED is a effective operation method treating lumbar disc herniation which is in line with the concept of minimally invasive and effective curative effect and can stand the survey and time inspection. With the development of surgical experience and endoscope technology, PTED can be used as effective surgical treatment on diversified lumbar disc herniation.
