Type 2 diabetes has become one of the major chronic diseases besides cancer and cardiovascular diseases. It has become one of the most serious problems affecting human’s health. Chronic high glucose induces vital organs including the central nervous system. Cognition refers to the process that brain receives external information, trans form into inner psychological activities and acquire the knowledge, including language, memory, calculation, understanding, judgment, visual spatial, execution and other aspects of the ability. Cognitive impairment includes mild cognitive impairment (MCI) and cognitive dysfunction (Dementia). The study found that, in patients with MCI increases by 1.5 times compared with non-diabetes patients, and the risk of dementia is even higher than 3 times normal. Diabetic encephalopathy is defined as different degrees of cognitive impairment caused by diabetes, which has become one of the complications of diabetes that can not be ignored.The etiology and pathogenesis of diabetes is complex. The pathogenesis of type 2 diabetic cognitive dysfunction also has not been fully elucidated. A variety of factors may also play a role in the body, interacting with each other and resulting in the injury of central nervous system. The study found that the following factors may be involved in the formation ofdiabetic encephalopathy:1.metabolic abnormalities in the brain; 2.unbalance homeostasis of Ca2+; 3. the deposition of Aβ,the phosphorylation of Tau protein, the injury of cerebral vascular; 4. Oxidative stress and cell apotosis, including the death receptor pathway, the caspase family, mitogen activated protein kinase pathway, nuclear factor kappa B, endoplasmic reticulum stress pathway, Bcl-2 family; 5.the renin angiotensin system (RAS) activation; 6.the hypothalamic pituitary adrenal axis (HPAaxis) in hyperthyroidism. These factors damage to the normal physiological function of neural cell and lead to type 2 diabetic cognitive dysfunction.Many reasons can result in cognitive dysfunction in type 2 diabetes, including personal factors, diabetes related factors, vascular factors, genetic factors and other risk factors. There is no uniform international standards for the diagnosis of cognitive dysfunction in type 2 diabetes. We require patients with type 2 diabetes, no cerebrovascular disease or other diseases which can cause the decline in cognitive function.We usually use the scale to evaluate the degree of cognitive impairment. Western medicine in the treatment of type 2 diabetic cognitive dysfunction requires the stable control of blood glucose. Based on the active treatment of the original diseases, we can use the drugs which can improve the symptoms of dementia.Diabetic encephalopathy is caused by diabetes. Long time diabetes influence brain or marrow, shang yin hao qi, qi yin liang xu. As it is deficiency in origin and excess in superficiality, so we should distinguish the actual situation.Type 2 diabetic cognitive dysfunction has become one of the most important diseases that we can not ignore. It affects seriously the patients’normal social work and daily life. With the improvement of technology, the pathogenesis of type 2 diabetic cognitive dysfunction will be more comprehensively and deeply understood.
