Treatment Of 53 Cases Of Proteinuria With Small Amount Of Nephritis In The Treatment Of Professor

Background:Proteinuria is one of the common clinical manifestations of chronic kidney disease, and to reduce the protein in the urine is still the main goal in chronic kidney disease.Patients with moderate amount or a large amount of albuminuria often use drugs such as hormones or immune inhibitors,while patients with a small amount of proteinuria often have a long clinical observation, or taking ACEI or ARB drugs.Many patients of a small amount of proteinuria come to seek treatment of traditional Chinese medicine,especially for occult nephritis, chronic nephritis.Objective:Professor Lv is China’s famous kidney disease expert, he has very rich clinical experience on the treatment for all kinds of chronic kidney disease with Chinese medicine.We evaluate the curative effect through comparing the changes before and after the treatment of 60 cases of occult nephritis or primary chronic nephritis patients.And based on 53 cases of pure traditional Chinese medicine,we try to analysis of TCM syndrome types, explore the medication features, and then summarize the experience of treating thought of professor Lv, and inherit the academic experience of traditional Chinese medicine.Method:Through the retrospective investigation and study, under the previous clinical summary from 2009 to 2014,the cases of occult nephritis (implicit kidney wind) or chronic nephritis (slow renal wind) in patients should have abnormal renal function and a small amount of proteinuria,such as the 24h urine protein quantitative between 0.15g and 2g, or urine protein in the urine routine qualitative+2 or less,and three times or more for the treatment.The total number of patients of occult nephritis or primary chronic nephritis is 98, while 38 cases are not complete.There are 13 patients with pathological diagnosis of IgA nephropathy (including 6 cases with hormone treatment)among the 60 patients, while 47 cases without pathological diagnosis (including 1 cases with hormone therapy).According to the case record information (general information, four diagnosis of traditional Chinese medicine, western medicine inspection, etc.), we evaluate curative effect of 60 patients.Through the analysis of 53 cases of pure traditional Chinese medicine treatment of patients, we try to reflect Professor Lv’s dialectical thought and formula experience.Summary:1 By comparison with 60 patients before and after their routine urine, we find that the treatment effective rate is 66.7%, and 53 cases of pure traditional Chinese medicine treatment effective rate is 67.9%.2 There are 34 cases of the actual situation and clip card,12 cases of pure empirical,14 cases of pure deficiency syndrome among 60 patients.In 53 cases of pure traditional Chinese medicine treatment,we find 31 cases of the actual situation and clip card,9 cases of pure empirical,13 cases of pure deficiency syndrome.3 Through detailed analysis of 53 cases,we should put great importance on the disease process of strive for good and evil, to correctly judge the virtual and the standard of primary and secondary aspects of clinical diagnosis.4 Professor Lv think the treatment of proteinuria should consider the disease, syndrome as well as the associated symptoms.Deficiency of Ben and repletion of Xie is the guidelines for the treatment of disease.During the treatment,Professor Lv pays attention to the insufficient of Qi.He also believes the Deficiency of Ben can conclude Yin deficiency and Yang deficiency,even Yin and yang deficiency.The repletion of Xie not only Shi evil but also blood stasis are important in the process of occurrence and progress of the disease.Besides, others evils should be treated during the dialectical medication. Therefore, professor Lv’s clinical prescriptions always contain tonify deficiency and eliminate pathogenic stagnation, especially JianPiBuShen, LiShiHuoXue.Professor Lv may choose more gentle herbs for patients for the short course of disease or the illness in remission after medication.But in view of the long course of disease, Professor Lv think evil poison would hurt the kidney obviously, and put forward the view of "renal collaterals lesions, the micro lump formation".At this time he add drugs like RuanJianSanJie, SouFengTongLuo, QingReJieDu. More curative effect can be obtained.Last but not the least, Professor Lv also think that we should avoid to use drugs that has been researched with Kidney injury.5 It is important for patients to keep a healthy diet, have a good mood,get a proper routine lifestyle, and do some moderate sports movement.
