Background:Lumbar disc herniation is a common and frequently-occurring disease in modern life, which is the main cause of low back pain and has seriously affected the work and life of patients. The radicular neuralgia is a disease that some harmful stimulation stimulates the spinal nerve roots (particularly for thedorsal root ganglion) and cause pain. Mechanical compression and inflammatory stimulation is the most commonly harmful stimulation of radicular neuralgia. For the lumbar disc herniation, with the disc herniation causing the compression on the nerve-root, the nucleus pulposus tissue and inflammatory factor also will stimulate and cause chemical injuries on the nerve-root, so root neuralgia symptoms is particularly common. A large number of clinical reports have concluded that acupotomy has a marked effect on the lumbar disc herniation, but the systematic research on the mechanism of its action remains to be further investigated, at the same time, the animal experiment on the lumbar disc herniation is rarely seen in the report.The acupotomy has a remarkable effect on the lumbar disc herniation and the literature research suggests acupotomy has advantages in the treatment of this disease.The experimental rats with the lumbar disc herniation was considered as the research object, adopting the method of tissue morphology and modern molecular biology methods, to study the analgesic effect on the lumbar disc herniation by the acupotomy in terms of the changes of local tissue morphology and peripheral pain related substances, in order to make it clear that the acupotomy has the analgesic action on the lumbar disc herniation and provide experimental support for the clinical treatment on the disease, to further promote the development of acupotomology.Objective:To observe the changes of Lsnerve-root in terms of tissue morphology, and the effect of acupotomy on 5-HT and (3-EP in serum of Rats with radicular pain of lumbar disc herniation, so as to make it clear that the acupotomy has the analgesic action on the lumbar disc herniation and provide experimental support for the clinical treatment.Methods:Fifty male SD rats were randomly divided into normal group, sham model group, model group, electroacupuncture group and acupotomy group, five groups in all and 10 rats in each group. The incubation period of shrinkage claw of each group was tested by PL-200; The pathological changes of L5 nerve root were observed by HE dyeing; Samples were took on eighth day after model, and the serum were taken to observe the changes of 5-HT and β-EP by ELISA.Results:1. The changes of pain threshold and pathological changes of L5 nerve root both suggest that lumbar disc herniationmodels were successfully constructed; acupotomy treatment on lumbar disc herniation can lighten the inflammatory response, lighten the inflammatory cell infiltration, and also reduce the abnormal proliferation of collagenousfiber, so as to promot the tissue repair.2. The results of comparison of the 5-HT among all groups were as follows:comparing with normal group(10.40±1.10 ng/mL), the model group(12.19±1.15 ng/mL) increased significantly, and the difference had statistical significance (P<0.01); comparing with model group (12.19±1.15 ng/mL), electroacupuncture group (11.05±1.36 ng/mL) decreased significantly, and the difference had statistical significance (P<0.01); comparing with model group(12.19±1.15 ng/mL), acupotomy group(10.73±1.11 ng/mL) decreased significantly, and the difference had statistical significance (P<0.01)The results of comparison of the β-EP among all groups were as follows:comparing with normal group(85.60±15.13 ng/L), the model group(108.14±11.08 ng/L) increased significantly, and the difference had statistical significance (P<0.01); comparing with model group(108.14±11.08 ng/L), electroacupuncture group(92.41±17.38 ng/L) decreased significantly, and the difference had statistical significance (P<0.01); comparing with model group(108.14±11.08 ng/L), acupotomy group(90.76±15.58 ng/L) decreased significantly, and the difference had statistical significance (P<0.01)The results of comparison of the 5-HT and the β-EP among all groups suggest that acupotomology can alter the regulation of 5-HT and the β-EP and give play to the role of suppressing pain.Conclusions:1. Acupotomology can effectively reduce the local inflammation of lumbar disc herniation and relieve inflammation lesion for the nerve-root, promoting tissue repair, so as to reduce or eliminate the pain.2. By the benign adjustment of pain substances in serum, acupotomology can lighten the stimulation to the nerve root caused by the pain substances and play a role of local analgesia.
