Survival Analysis Of Malignant Tumor In A Community In Chaoyang District Of Beijing From 2006 To 2011 And Investigation On Quality Of Life Of Gynecological Malignant Tumor And Its Qualitative Research

Objective:Based on the followed-up monitoring data of malignant tumor collected during 2006 to 2011 from residents of Xiao guan Community, Chao yang District, Beijing, the survival analysis of:malignant tumor patients in the Community is carried out and the survival rate of malignant tumor among the Community residents is calculated. The data can provide the foundation of integrated control of the malignant tumor. Meanwhile, the patients of specific kinds gynecologic malignant tumor, which are listed as C51~C57 and D06 in the ICD-10, are screened out. For patients who are still alive after the deadline of the observation, evaluation of quality of life is carried out under consent. It may contribute to the community care to the gynecologic malignant tumor patients alive. Moreover, qualitative interview is demonstrated to patients with consent, to identify their needs and specialize the policy implementation and medical care for them.Methods:Based on the clinical cases registered during 2006 to 2011 in the medical system of Xiao guan Community, the survival rate of the community is analyzed with Life Table method. The survival rates of patients of different age, different gender and different kinds of malignant tumor are compared with the help of Meier-Kaplan method. Furthermore, the Cox model is built to inspect the impact of age and gender on the survival rate. In the comparison of the three common kinds of gynecologic malignant tumor, ANOVA is applied if the data follows normal distribution and Kruskal-Wallis test is applied otherwise. The recordings of qualitative interview is transcribed into manuscripts for coding.Results:The one-year, five-year and six-year survival rates of malignant tumor patients in Xiao guan Community were 97.33%,85.96% and 84.85%, respectively; the six-year survival rates of patients of age groups,<40,40~,50~,60~,70~,80~, were 96.55%,100.00%,98.87%, 93.98%,80.98% and 61.89%, respectively; the six-year survival rates of males and females were 76.33% and 91.54%, respectively; the six-year survival rates of breast(female), lung and bronchus, rectum, bladder, kidney(except renal pelvis), colon, prostute(male), thyroid gland, liver and intrahepatic bile ducts, stomach were 96.43%,59.90%,90.48%,91.67%,100.00%,95.65%, 89.39%,100.00%,54.51%,93.10%, respectively. In Xiao guan Community, the patients of cervical carcinoma, ovarian carcinoma and endometrial carcinoma shows no significant difference in the quality of life and the functional status. Among the symptom scales, nausea & vomiting and constipation show a significance of 0.05 and may show significant difference under a larger sample. The mean values of nausea & vomiting scale and constipation scale are close to 100 among the patients of endometrial carcinoma, indicating that they suffer from a worse quality of life. Since there is only one interviewee in the qualitative interview, the result is not representative.Conclusion:The survival rates of malignant tumor patients decrease as the age of the patient increases. The survival rate of females is higher than males. The survival rates differ as the malignant tumor site differs, with the lowest values obtained from patients of lung carcinoma and liver carcinoma. No significant difference in the survival rate is found in different genders. While the survival rate of prostate carcinoma (male) is fair, that of breast carcinoma (female) is high despite the large number of patients. The patients of gynecological malignant tumor show no significant difference in the quality of life, but nausea & vomiting and constipation is more common among the patients of endometrial carcinoma, indicating a worse quality of life. In the future work in Community, qualitative interview should be included with more interviewees to gain substantial opinions and advices.
