This article mainly includes two aspects of clinical and experimental study of the Shugan Yishen decoction for HER2 high expression LuminalB TAM resistance breast cancer patients.The first part:The clinical study of Shugan Yishen decoction treat for HER2 high expression LuminalB TAM resistance breast cancer. Objective:To explore the clinical effect of Chinese medicine Shugan Yishen decoction on HER2 high expression TAM resistant breast cancer patients.Methods:50 cases progression breast cancer patients after tamoxifen treatment were collected in this study. The western medicine group select pure endocrine therapy while the others use Chinese decoction treatment on the basis of western medicine.Both groups have not taken the targeted therapy. The main outcome of the study was the progression-free survival. The secondary outcome was the quality of life score score of syndromes and the efficacy of TCM syndromes. The data were analyzed by using the SPSS17.0.Results:1. Progression-free Survival:Cox regression multivaritate analysis shows: metastatic sites, groups, previous Herceptin treatment are independent influence factors of PFS (P<0.05). The western medicine group’s median PFS:4months, while combined group’s median PFS:5.5 months, the two groups have significant difference (P<0.05).2. Quality of life scale:(1) Physical status score:comparison among groups,8 weeks after treatment, two groups in terms of physiology score higher than baseline, the differences were statistically significant (P<0.05), and 16 weeks after treatment, two groups have significant differences (P<0.01). comparison within groups, Western medicine group, compared wi th before treatment,8 weeks after treatment, the score differences were statistically significant (P<0.05); but 16 weeks after treatment, compared with before treatment, the score has no difference (P> 0.05), and compared with 8 weeks after no difference (P>0.05); combined group, compared with before treatment,8 and 16 weeks after treatment, the differences were statistically significant (P<0.01); and 16 weeks and 8 weeks after treatment compared also have difference (P<0.05).(2)Social/family status score:comparison among groups,8 weeks after treatment, two groups in terms of physiology score higher than baseline, but the difference was insignificant (P>0.05),16 weeks after treatment, the two groups still have no obvious difference (P>0.05).comparison within groups, Western medicine group, compared with before treatment,8 weeks after treatment, the score differences were statistically significant (P<0.05); but 16 weeks after treatment, compared with before treatment, the score has no difference (P>0.05), and compared with 8 weeks after no difference (P>0.05); combined group, compared with before treatment,8 weeks after treatment, the score differences have no statistical significance (P>0.05); 16 weeks after treatment, compared with before treatment, the score differences were statistically significant (P<0.05),while compared with 8 weeks after treatment.there have no differences (P>0.05).(3)Emotional status score:comparison among groups,8 weeks after treatment, the differences between the two groups have no statistical significance (P>0.05), while 16 weeks after treatment, the differences were statistically significant (P<0.01).comparison within groups, western medicine group compared with before treatment,8 and 16 weeks after treatment, the score difference had statistical significance (P<0.05), while 16 weeks after treatment, compared with 8 weeks after treatment, there have no differences (P>0.05); combined group, compared with before treatment,8 and 16 weeks after treatment, the differences were statistically significant (P<0.01); and 16 weeks and 8 weeks after treatment compared also have difference (P<0.01).(4)Function status score:comparison among groups,8 weeks after treatment, the differences between the two groups have no statistical significance (P>0.05), while 16 weeks after treatment, the differences were statistically significant (P<0.01).comparison within groups, western medicine group compared with before treatment,8 and 16 weeks after treatment, the score difference had statistical significance (P<0.05), while 16 weeks after treatment.compared with 8 weeks after treatment, there have no differences (P>0.05). combined group, compared with before treatment,8 and 16 weeks after treatment, the differences were statistically significant (P<0.05); and 16 weeks and 8 weeks after treatment compared also have difference (P<0.01).(5)Additional functional score:comparison among groups,8 weeks after treatment, the differences between the two groups have no statistical significance (P>0.05), while 16 weeks after treatment, the differences were statistically significant (P<0.01).Comparison group, western medicine group compared with before treatment,8 and 16 weeks after treatment, the score difference had no statistical significance (P>0.05), and 8 weeks compared with 16 weeks also have no difference (P>0.05); combined group, compared with before treatment,8 and 16 weeks after treatment, the differences were statistically significant (P<0.01); and 16 weeks and 8 weeks after compared also have difference (P<0.01).(6)Total score of the scale:comparison among groups,8 weeks after treatment, the differences between the two groups have no statistical significance (P>0.05), while 16 weeks after treatment, the differences were statistically significant (P<0.01).comparison within groups, western medicine group compared with before treatment,8 and 16 weeks after treatment, the score difference had statistical significance (P<0.05), while 16 weeks after treatment, compared with 8 weeks after treatment, there have no differences(P>0.05); combined group, compared with before treatment,8 and 16 weeks after treatment, the differences were statistically significant (P<0.01); and 16 weeks and 8 weeks after compared also have difference (P<0.01).3. TCM syndrome score:8 weeks after treatment, western medicine group improved significantly in 2 patients, partial improvement 10 cases, no improvement of 7 cases, total effective rate 63.2%, the combined group improved significantly in 12 cases, partial improvement 13 cases,without improvement in 4 cases, total effective rate was 86.2%. The difference between the two groups was statistically significant (P<0.05).16 weeks after treatment, the western medicine group improved significantly 3 cases, some improved in 6 cases, no improvement in 3 cases, total effective rate 75.0%, the combined group improved significantly in 14 cases, partial improvement 4 cases, without improvement in 1 case, the total effective rate was 94.7%. The difference between the two groups was statistically significant (P<0.05).Conclusion:1. Shugan Yishen decoction Combined with western medicine treatment can prolong patients’progression free survival2. Shugan Yishen decoction Combined with western medicine treatment can improve the patients’clinical symptoms and quality of life.The second part:The effect of Shugan Yishen decoction towards TAM resistant cell line about ER、HER2 expression regulation Objective:To research the regulation of Shugan Yishen decoction towards the expression ofERand HER2 of TAM resistant cell line, and clarify its resistance mechanism. Methods:1.Preparing drug contained serum; 2. Using Immunocytochemistry and immunof luorescence detection to observe influence of serum towards ER α and HER2 expression of MCF-7 LCC9 cells; 3. Using immunoprecipitation to observe the interaction between ER αand HER2.Results:Immunocytochemistry and immunof luorescence results showed that compared with the blank group, the TAM group ER alpha expression have no obvious difference (P> 0.05). Combined group and Chinese medicine group ER alpha expression was increased (P<0.05), HER2 protein expression decreased (P<0.05) and the expression of ER alpha in combination group higher than other groups (P<0.05) and HER2 expression is lower than the other groups (P<0.05). There was no direct interaction between HER2 and ER by immunoprecipitationConclusion:Shugan Yishen Decoction can effectively increase the expression of ER, decrease the expression of HER2 to the synergistic effect of treatment resistant, while there have no direct interaction between ER and HER2 in the drug-resistant cell line.
