Objectives:The aim of this retrospective study was to prove the effectiveness of heat-cleaning and detoxicating encircling method in infected diabetic foot patients and explore its applied range. We also wanted to see if the local TCM syndrome and systematic TCM syndrome were agree with each other and the relationship between them.Methods:First we chose cases who had received the treatment of heat-cleaning and detoxicating encircling method among all the diabetic foot patients hospitalized in the endocrinology unit and vascular surgery unit from Jan.2007 to Jun.2014.Then collected the patient’s information and clinical data from their medical records including general information, the results of bacteria culture from the secretion of foot ulcer, local manifestation,general symptoms. Statistics analyzing were done in the end of the study using SPSS 19.0 statistical software.Results:1.The average duration of hospital stays of the patients received the treatment of heat-cleaning and detoxicating encircling method is 24.22±14.80 days.The average hospitalization cost was 12900.80±4753.94 yuan. When compared to similar domestic research,our study has significant advantage (p< 0.05).2. The total efficiency of the treatment is 79.9%,We also find heat-cleaning and detoxicating encircling method is suitable for patients who suffer from a local TCM syndrome of noxious heat Syndrome or damp-heat Syndrome in classification of Wagner grade two or grade three(p< 0.05). Heat-cleaning and detoxicating encircling method is more effective in Gram-Positive Bacterial Infected group than in Gram-Negative Bacterial Infected group,and there is statistical difference between the two groups(p< 0.05). Our study shows that S. aureus(18.4%)is the gram-positive bacteria most likely found in diabetic foot lesions, followed by enterococcus faecalis(11.2%); while E.coli(9.6%)is the most frequent gram-negative bacteria found in the study, followed by P seudomonas aeruginosa(7.2%). Quinolones(29.33%) is the most commonly used antibiotics, followed by Penicillins and Cephalosporins(22.67%). The average duration of antib iotic treatment is 8.47±4.72 days.3. Noxious heat Syndrome (31.6%)and or damp-heat Syn drome(39.5%)are the most commonly find local TCM syndrome in our study,which go f ar beyond cold-dampness Syndrome (5.3%) and syndrome of yang deficiency and b1 ood stasis(15.8%). The distribution of proportion in systematic TCM syndrome are as fol lows Toxic heat syndrome (50%),syndrome of blood stasis (27.6%),blood deficiency syndrome (26.3%),dampness-phlegm syndrome (17.1%),Qi deficiency syndrome (10.5%), Yin deficiency syndrome (9.2%),Yang deficiency syndrome (6.6%), Energy Stagnant syndrome(1.3%).There are patients whose local TCM syndrome and systematic TCM syndrome are not agree with each other,but we have not found obvious correlation about local TCM syndrome and systematic TCM syndrome.
