Objective:To observe the effect of sevoflurane on muscle relaxation in elderly patients using mivacurium.Materials and methods:Sixty patients aged from65-75with ASA â… -â…¡ were enrolled in our study. Patients with other muscle relaxant drugs allergy, neuromuscular diseases, liver and kidney dysfunction, serious acid-base imbalance, or severe disorder of plasma electrolytes were excluded. All the sixty patients underwent selective operation with general anesthesia. The patients were randomly divided into1.0minimum alveolar concentration (MAC) of sevoflurane group (group A),1.3MAC of sevoflurane group (group B) and propofol group (C group), with20cases in each group. Anesthesia was induced by propofol (2mg/kg) and sufentanil(0.4ug/kg). Mivacurium(0.15mg/kg) was given after the patients losting their consciousness, and when the T1reached the maximum block the endotracheal tube was inserted, followed by mechanical ventilation with Drager anesthesia machine. Group A and group B underwent intravenous infusion of propofol2-5mg/kg/h, plasma target controlled infusion of remifentanil4ug/ml, and continuous inhalation of sevoflurane to maintain the concentrations of sevoflurane were1.0MAC and1.3MAC at the end of breath. Group B underwent intravenous infusion of remifentanil4-10mg/kg/h, plasma target controlled infusion of propofol4ug/ml. TOF-Watch SX (Organon) acceleration monitor for muscle relaxation was selected to monitor muscle relaxation. The following indicators were observed:(1)The effect time:duration from the muscle relaxant producing the greatest muscle relaxant effect to T1returning to25%of the control time.(2) The recovery index: T1recovered from the25%of the control time to the75%of the control time.(3) The maintenance of T1is the pumping speed for5%-10%muscle relaxants.Results: Compared with group C, the effect time, the time of TOF returning to25%and75%, and the recovery index of group A and group B were significantly prolonged. Compared with group A, the effect time, the time of TOF returning to25%and75%, and the recovery index of group B were significantly increased.Conclusion:Inhalation of sevoflurane can enhance the muscle relaxation effect and prolong the effect time of mivacurium in eiderly patients. And inhalation of1.3MAC sevoflurane can significantly enhance the muscle relaxation effect of mivacurium.
