| Objective: This study is to investigate the prevalence of osteoporosis(OP)and the influencing factors of OP in young people in County,southern Xinjiang,China.Multidimensional analysis of the influencing factors of osteoporosis,in order to provide a theoretical basis for early prevention and treatment.IL-1R2,serves as a negative regulator of IL-1 signaling,is involved in the pathogenesis of osteoporosis.This study aimed to determine the correlation between IL-1R2 polymorphism and osteoporosis susceptibility among the Chinese Han population.Methods:1.Two-stage random cluster sampling was used to select subjects from 102 natural villages in 3 towns and 4 townships in the Moyu County.Bone density was measured.OP was diagnosed based on the T-score value.The BMI standardized prevalence was calculated.Multivariate logistic regression was used to analyze the influencing factors of osteopenia and OP,and the binary logistic regression was used to analyze the influencing factors of OP in the young people(< 50 years old).2.The influencing factors of 4979 people diagnosed with osteoporosis and non-osteoporosis in the first part were analyzed.Analyze the influence of diet,daily habits,body fat and other related factors on osteoporosis,first one-factor analysis,then logistics regression analysis,and finally through the establishment of theoretical models,exploratory factor analysis,verification factor analysis to construct structural equation models;3.We recruited 594 osteoporosis patients and 599 healthy controls.Six single nucleotide polymorphisms(SNPs)in IL-1R2 were selected for genotyping using Agena Mass ARRAY platform.Odds ratio(OR)and 95% confidence interval(CI)was calculated through logistic regression analysis with adjustment for age and sex.Linkage disequilibrium analysis was plotted by Haploview v4.2.Multifactor dimension reduction(MDR)was performed to estimate the SNP-SNP interaction of IL-1R2 variants.Results:1.Totally 11502 qualified cases based on the bone density test were included.The overall prevalence of OP in the Moyu County was 18.52%(95%CI 17.82-19.24),and the prevalence of osteopenia was 56.71%(55.80-57.62).2.The OP prevalence for all subjects,males,and females increased with age(P<0.001),while the trend in the males showed a higher prevalence at lower ages.3.The prevalence of osteoporosis in men aged 30 years < 11.04%(8.47-14.26)is higher than that in women,the prevalence of osteoporosis in overweight men aged 30 to 39 years and40 to 49 years old is higher than that in overweight women,the prevalence of osteoporosis in normal and overweight women aged 60 to 69 years and 70 years old is higher than that in men,and the prevalence of osteoporosis in women with normal weight is higher in the 50-59 years old group and ≥ 80 years old group.4.Univariate findings that the factors affecting osteoporosis include:(1)female,high education,fracture history,and previous chronic diseases in the osteoporosis group,P<0.05;(2)household fuel is firewood/carbon,smoke-free equipment,and the proportion of indoor heating in the winter is higher in the osteoporosis group,P<0.05;(3)the BMI,waist circumference,body fat percentage,obesity degree is higher in the osteoporosis group,non-fat volume,muscle mass score,body water content,and basal metabolism are lower,P<0.05;(4)The proportion of vitamin D supplementation and calcium and vitamins in the osteoporosis group is higher than that of eggs and products,cow and goat milk,yogurt and other dairy products,P<0.05;(5)the proportion of tea,carbonated drinks and other drinks per week in the osteoporosis group is higher,the frequency of never eating rice and grains is higher,the proportion of eating meat,fresh vegetables,dried fruits,etc.is lower,the proportion.5.Age and menopause were the most important factors affecting OP,while the vitamin D supplementation may only be effective in young people.Overweight or obesity could reduce the prevalence of OP in the elderly and increase its prevalence in young people.Multivariate logistic regression showed that aging,low body mass index,and menopause were risk factors for OP or Osteopenia,and the overweight and vitamin D supplementation were protective factors for OP or Osteopenia.The risk factors for osteoporosis in people less than 50 years old include overweight,obesity,and male.6.The fitting indices of structural equations are: RMSEA=0.044,NFI=0.973,RFI=0.962,IFI=0.977,TLI=0.967,CFI=0.977,all >0.9 models fit well.7.The structural equation shows that: general condition,body fat,calcium and exercise are the factors that affect osteoporosis,respiratory exposure,daily diet,through the interaction with general condition,body fat,calcium and exercise,indirectly affect osteoporosis,The normalized path coefficient of the general situation is the highest,followed by body fat,and finally calcium and exercise,that is,the general situation has the greatest impact on osteoporosis.8.Our result revealed that rs11674595(OR = 1.86,P = 0.020),rs2072472(OR = 1.26,P =0.019)and rs4851527(OR = 0.78,P = 0.007)were related to the risk of osteoporosis.Moreover,the contribution of IL-1R2 polymorphisms to osteoporosis risk presented age,sex and BMI difference.9.We found the relationship of Trs11674595Ars4851527(OR = 0.80,P = 0.015),Crs11674595Grs4851527(OR = 1.22,P = 0.043)and Ars3218977Grs2072472(OR = 1.25,P =0.022)haplotypes to osteoporosis occurrence,and a potential accumulated effect of IL-1R2SNPs(testing accuracy = 0.5783 and CVC = 10/10)on osteoporosis susceptibility.Conclusion:1.The prevalence of OP in Moyu County is higher than those throughout China and the world.People under 50 years are more likely to suffer from OP,especially for the females.It is necessary to pay attention to the prevention of OP in this area to reduce the social burden caused by osteoporotic fractures.2.Age and menopause are the most important reasons for osteoporosis.Calcium and vitamin D supplementation and BMI have different effects in different age groups,and the important effect of visceral fat cannot be ignored.3.IL-1R2 polymorphisms(rs11674595,rs4851527,rs2072472 and rs3218977)might contribute to osteoporosis risk among the Chinese Han population.Our finding may increase our understanding of the effects of IL-1R2 polymorphisms on the predisposition of osteoporosis. |
