| Objective:In order to improve the diagnostic accuracy of extraocular myoparalysis,increase the data of the condition changes and bring acupuncture’s superiority into full play in treatment,develop support system which can help the doctor for acupuncture treatment and improve decision-making ability in acupuncture treatment,besides,put this system into clinical application to evaluate the curative effect.Methods:1.Built the mechanical structure of eye movement disorders detect instrument,including head fixed institutions and transmission mechanism for the movement of Hess screen.2.Designed the software system,Including HESS curve drawing and a thesis proposes a new method which took the error caused by the distance between patient’s both eyes into consideration.3.The decision support algorithm of eye movement disorders was wrote which was based on SVM(Support vector machine).PSO(Particle Swarm Optimization)is used to optimize SVM parameters.Clinical records were analyzed to improve the treatment effect.4.120 Patients were divided into 2 groups,one group accepted eye-acupuncture treatment,another traditional acupuncture treatment,Double vision Angle、Eye movement distance and Quality of Life Scales were applied in the evaluation before、after and Different time points during the treatment,besides,safety and adverse reactions we reevaluated before and after the treatment.Results:1.Detection and computer-aided diagnosis were executed in HMU(Harbin Medical University),the result showed they were trustful.Evaluation system for eye movement nerve palsy not only intelligently provide computer aided diagnosis,but also automaticly generate double vision angle,which can guide clinic acupuncture treatment.The system,with technique Feature of easy operating,accuracy and good reliability and excellent repetition,has good clinical value.2.The effective rate and Clinical control rate of eye-acupuncture group were much higher than traditional acupuncture group(P<0.05).The recovery of total diplopia angle of eye-acupuncture group were better than traditional acupuncture group at the end of 1.2.3 Period of treatment(P<0.05),while at the end of 1 Period of treatment was the same as traditional acupuncture group at the end of 2 Period of treatment(P>0.05).while at the end of 2 Period of treatment was better than traditional acupuncture group at the end of 3 Period of treatment(P<0.05).In the aspect of diplopia angle,all extraocular muscles of eye-acupuncture group were better than the traditional acupuncture group at the end of 1.2.3 Period of treatment(P<0.05).External rectus,medial rectus muscle,superior rectus,the rectus,and inferior oblique muscles of eye-acupuncture group at the end of 1 Period of treatment were the same as traditional acupuncture group at the end of 2 Period of treatment(P>0.05).In the aspect of Eye movement distance,External rectus,medial rectus muscle,superior rectus,the rectus,and inferior oblique muscles and superior rectus of eye-acupuncture group were better than the traditional acupuncture group at the end of 1.2.3 Period of treatment(P<0.05),levator palpabrae muscle of eye-acupuncture group was better than the traditional acupuncture group at the end of 1 Period of treatment(P<0.05),while there was no difference between the end of 2.3 Period of treatment(P>0.05).Eye movement distance of External rectus,medial rectus muscle,superior rectus,the rectus,and inferior oblique muscles of eye-acupuncture group at the end of 1 Period of treatment were the same as traditional acupuncture group at the end of 2 Period of treatment(P>0.05).Eye movement distance of Superior oblique muscle of eye-acupuncture group at the end of 1 Period of treatment was better than traditional acupuncture group at the end of 2 Period of treatment(P<0.05).Before and after treatment,the quality of life scale of eye-acupuncture group was superior to the traditional acupuncture group(P<0.01).There were no serious adverse events during acupuncture,some slight adverse reaction was easy to eliminate by corresponding measures.Conclusion:1.Eye movement nerve palsy evaluation system can automatically record the result of double vision Angle and diplopia distance to realize the aim of data storage and quantitative indicators.2.Eye movement nerve palsy evaluation system,with function stability,can provide important effect to clinical diagnosis,assessment and guidance in acupuncture treatment.The detection results are consistent with the traditional test results and are in conformity with clinical symptoms.3.The method of eye-acupuncture therapy to eye movement nerve palsy caused by diabetes mellitus and cerebral infarction is superior to the traditional acupuncture therapy. |
