Objective:To investigate the influencing factors of the oculomotor nerve palsy(OMNP)related to intracranial aneurysm. Methods:Analyze the clinical data from 162 patients with intracranial aneurysm in our hospital from January 2009 to December 2016 retrospectively. The possible affecting factors include sex, age, location, site and size, subarachnoid hemorrhage, multiple aneurysms, time from symptom onset to treatment, hypertention,et al.We measured the diameter of intracranial aneurysms by Siemens' digital subtraction angiography and used maximum diameter as the size of the aneurysms.Univariate analysis and binary Logistic regression analysis were used to evaluate the possible affecting factors of OMNP.Also we use the same method to study possible affecting factors of the degree of OMNP.Results:The univariate analysis showed that sex(P=0.01),site of aneurysm(P<0.01)and size of aneurysm(P<0.01)were significantly different between patients with oculomotor nerve palsy and patients without OMNP which is related to intracranial aneurysm. And site of aneurysm(P =0.02),time from symptom onset to treatment(P=0.03)were significantly different between patients with total OMNP and patients with partial OMNP which is related to intracranial aneurysm. The logistic regression analysis showed that site of aneurysm(OR=10.32,P<0.01)and size of aneurysm(OR=10.2,P<0.01)were influence factors of OMNP.There are correlations between time from symptom onset to treatment(OR=4.92,P=0.04)and OMNP.Conclusion:The site and and the size of intracranial aneurysm were influence factors for OMNP.There are correlations between time from symptom onset to treatment and OMNP.Among aneurysms with OMNP,posterior communicating artery aneurysms(PcoAA)and cavernous sinus aneurysm were the most common.PcoAA was risk factors for OMNP.Aneurysm was inclined to happen when the diameter surpass 6mm.When the Time from symptom onset to teatment less than 20 days,it was more likely to show complete OMNP.On the contrary,it was more likely to show partial OMNP.The most common clinical manifestation of OMNP were ptosis and restriction of eye movement.
