Screening,Clinical Features,and Prognostic Analysis Of Liver Cirrhosis-related Hepatocellular Carcinoma

Objective:To explore the impact of the screening interval on cirrhosis-related hepatocellular carcinoma(HCC)detection and to analyse the clinical features and prognosis of HCC.Materials and methods:We recruited 3,000 patients diagnosed with liver cirrhosis,who had been treated at the First Affiliated Hospital of Anhui Medical University from January 2016 to January 2020.The time of admission was divided into 3-(group A,539cases),6-(group B,1,012 cases),and 12-month screening groups(group C,1,449cases).We compared the detection rate of HCCs in each group and analysed the clinical features and prognosis of the patients with HCC.Results:We detected a total of 124 HCC cases,including 32,38,and 54 cases in group A,B,and C.41 cases of small HCC:21,14,and 6 cases in group A,B,and C,respectively.The detection rate of HCC was 5.9%(32/539)in group A,which was significantly higher than that in groups B and C(?~2=5.397,P=0.020).The detection rate of small HCC was3.9%(21/539)in group A,which was significantly higher than that in groups B and C(?~2=31.186,P<0.001).Male 103 cases,Chronic hepatitis B 90 cases accounted for83.1%,72.6%,respectively.Single small,right liver lobe,and We detected vascular invasion and lymph node metastasis in one and three cases,respectively.alpha-fetoprotein-negative HCCs and s HCC accounted for 54.8%,68.3%,respectively.The average survival time of HCC patients in group A was(34.13±14.26)month,which was significantly higher than that in groups B and C(34.13±14.26 vs.27.84±10.93 vs.22.43±12.58)(F=8.797,P<0.001).The average survival time of patients in the small HCC group was significantly higher than that in the non-small-HCC group(35.68±12.95 vs.22.87±11.42months)(t=5.623,P<0.001).Conclusions:Screening patients with a history of liver cirrhosis at intervals of 3 months can increase the detection rate of HCCs and s HCC.Male,over 50 years old,hepatitis B virus infection were risk factors for HCC in cirrhotic patients.The small HCC lesions predominantly comprised single and right lobular lesions.Lymph node metastasis and vascular invasion were rare.screening interval,tumor diameter,AFP and treatment modalities all affect the prognosis of HCCs.Early detection can provide more patients with an opportunity for radical treatment and prolong their survival.
