Chronic diseases,especially for diabetes,can have a profound impact on the health and quality of life of elder,not to mention the financial burden that is often associated with long-term illness.Pertinent and prompt policy solutions are necessary to ensure fiscal and well-being of aging populaiton.The wide application of emerging information technologies such as big data,artificial intelligence,cloud computing,Internet of Things,and 5G in the health field has created new opportunities for mhealth to meet the challenges posed by an aging society.Mobile phone applications(apps)have been shown to successfully facilitate the self-management of chronic disease and a large number of studies have been carried out to m Health adoption and usage.However,less attention has been paid to the value of mobile health care and lack of causal evidence support.Three completely randomized field experiments were conducted to reveal the causal effect of mhealth usage on the improvement of health out come and reduces medical costs,and proposes short-term and long-term monetary and non-monetary incentives effect in m Health.Also,we find that privacy concerns in mobile health care are issues that cannot be ignored in the design of personalized services.Finally the theoretical framework based on self-determination theory has been established.The main research contents,methods and findings are as follows:1.In view of the impact of mhealth on the health outcome and economic value of patients,a strictly randomized field experiment was successfully implemented.In the experiment setting,the platform effect was considered by using two control groups,and the value of personalized services for mobile medical users was considered.We causality proved that the use of m Health platform can significantly reduce the blood glucose of patients over time and reduce patient visits hospital and medical costs.Intreastingly,It was found that personalized services were no dignificant intervetion effect.2.Based on self-determination theory,we study the effect of monetary incentives.A field experiment with fixed monetary incentives and gamified monetary incentives was established.The results causaly prove that monetary incentives have a significant positive effect on improving usage,and that gamified method performs better.In order to study the long-term effect of monetary incentives,the instrumental variable method was used to verify the causality of the withdrawal of monetary incentives.The resluts show that the effects disappeared after withdraw the monetary incentives.Subsequently,based on the Heckman 2-stage SLS model,we explored that monetary incentives impact on patients' health outcome.3.Based on self-determination theory,we study the effects of non-monetary incentives.A field experiment refered to Quantify Self and social norms were established.The results prove that QS has a significantly positive effect on improving usage,and will last long even when the incentive was stopped.Interestingly,social norms have no significant incentive effect in the m Health scenario.Subsequently,based on the Heckman2-stage SLS model,we explored that non-monetary incentives have a positive impact on the patient's health outcomes.4.Based on self-determination theory,based on interviews and questionnaires,we conducted a natural experiments.Based on the privacy protection settings,we found that privacy protection setting will allow users to choose more personalized services and fewer non-personalized services.When privacy is not well protected,privacy concerns will lead to the invalidation of personalized service settings.At the same time,the severity of the disease will negatively strengthen this relationship.The research results of this paper further deepen the understanding of the value and incentive effect of m Health.The three randomized field experiments and one natural experiment in this paper propose a theoretical framework of mobile health useage and behavior change.It is of great theoretical and practical significance to scientifically perfect the current theoretical framework of m Health design.
