Research On Methods For Two-side Matching Of Surgeons And Patients In Elective Surgery

Surgery is the main medical service provided by the hospital for patients,and it is the most important and intensive medical activity in the daily operation and management of the hospital.According to the urgency of operation time,the operation can be divided into three categories:elective operation,limited operation and emergency operation.Among them,elective surgery refers to the operation that can be performed at the right time.The grasp of the operation time does not affect the therapeutic effect.It allows sufficient preoperative preparation or observation,and then chooses the most favorable time for the operation.Elective surgery accounts for 86%of the three types of surgery.Because of the large proportion of elective surgery,patients,as the demand for elective surgery,their satisfaction with elective surgery will directly affects the reputation of the hospital.As the implementer of elective surgery,surgeons are the most valuable medical resources in hospitals.Their proficiency in surgery will not only affect the efficiency of surgery,but also affect the utilization rate of medical resources and operating costs.Therefore,how to obtain a reasonable matching between surgeons and patients and achieve a win-win effect between surgeons,patients and hospitals is an urgent problem to be solved.At present,the research on two-sided matching of surgeons and patients in elective surgery is still scattered,and has not been systematically studied.Therefore,aiming at the problem of the two-sided matching of surgeons and patients in elective surgery,this paper studies related theories and methods,and mainly completes the following three aspects of research work:(1)The problem of two-sided matching of surgeons and patients in elective surgery is refined.The characteristics and common preference information of two-sided matching between surgeons and patients in elective surgery are analyzed and summarized.The general concept of surgeons/patients matching in elective surgery is defined.The differences and relations between the related concepts in surgeons/patients matching and those in the corresponding traditional one-to-many two-sided matching problem are analyzed and compared.Furthermore,the importance and necessity of further research on method of the surgeons/patients two-sided matching in elective are concluded.(2)The determination of the surgeons/patients matching scheme in elective surgery is studied.Considering that patients may provide preference order information,multi-attribute evaluation information and pairwise comparison preference information when evaluating surgeons,in order to guarantee the stability,satisfaction and Pareto effectiveness of the surgeons/patients matching scheme,the following methods are proposed respectively:(A)The determining method of surgeons/patients matching scheme based on preference order information;(B)The determining method of surgeons/patients matching scheme considering attribute priority;(C)The determining method of surgeons/patients matching scheme based on pairwise comparison preference information.(3)The adjustment of the surgeons/patients matching scheme in elective surgery is studied.Considering that patients or surgeons may cancel the operation,the most representative adjustment of the surgeons/patients matching scheme are studied,i.e.,the adjustment of surgeons/patients matching scheme for selective surgery when some patients cancel surgery and the adjustment of surgeons/patients matching scheme for selective surgery when surgeons cancel some surgeries.The concepts of surgeons/patients matching schemes when patients or surgeons cancel surgery are given respectively.The differences and relations between the adjustment of surgeons/patients matching scheme when patients or surgeons cancel surgery and the related properties of the surgeons/patients matching scheme in elective surgery are analyzed and proved.On this basis,the basic adjustment rules that should be followed when adjusting the initial surgeons/patients matching scheme under different circumstances are given.In order to ensure the effectiveness of the adjustment of the surgeons/patients matching scheme,based on the Deferred Acceptance(DA)algorithm,the following methods are proposed:Patients canceling-deferred acceptance(PC-DA)algorithm and Surgeons canceling-deferred acceptance(SC-DA).Based on the problem of surgeons/patients matching in elective surgery,this paper proposes corresponding methods for surgeons/patients matching in elective surgery according to different situations.It enriches and develops the theory and method oftwo-sided matching,and provides reference value for further research on the theory and method of surgeons/patients matching in elective surgery.The method proposed in this paper can better solve the problem of surgeons/patients matching in elective surgery,and can be extended to other two-sided matching problems with the characteristics of such problems,such as the two-sided matching between multi-skilled workers and jobs,the two-sided matching between housekeepers and employers,etc.
