Clinical Study Of Pediatric Wind-heat Cough By Tuina Based On Data Mining Of Literature

Objective: To explore the regulation of children with wind-heat cough by Tuina based on data mining technology and reconstruct a new prescription.To evaluate the clinical efficacy of the new prescription by a randomized control trial,in order to provide a safe r and more effective prescription for children with wind-heat cough by Tuina.Methods: 1.Literature Study: Ancient and modern literature,related to children with cough,was consulted and sreened.Then literature database was established and analyzed from acupoint's frequency,distribution,cluster analysis,correlation analysis and visual centrality by software such as SPSSS tatistics23.0,SPSS Modeler18.0,SATI,UCINET,etc,to explore the acupoints selection rules of wind-heat cough by Tuina in ancient and modern times and preliminary constructed a new prescription.2.Clinical Study: 105 children with wind-heat cough who met the diagnostic and inclusion criterias were randomly divided into treatment group(n=35),control group 1(n=35)and group2(n=35).Treatment group: Acupoints: Feijing,Nei Bagua,Li to Qian,Kan and Li,Gen to Li,Xinjing,Nei Laogong;Operations: Clearing Feijing,delivering Nei Bagua clockwise,pushing from Li to Qian,pushing back and forth between Kan and Li,delivering Gen to Li,pinching Xinjing,kneading and pinching Nei Laogong.Control group 1: Acupoints: Xiaotianxin,Banmen,Feijing,Xiaohengwen,Tianheshui;Operations: Kneading Xiaotianxin,clearing Banmen,clearing Feijing,kneading Xiaohengwen,clearing Tianheshui.Control group 2:Acupoints: Feijing,Ganjing,Nei Bagua,Sihengwen,Tianheshui;Operations:Calming Ganjing and clearing Feijing,delivering Nei Bagua clockwise,pushing Sihengwen,clearing Tianheshui.All children were treated once a day,7 times as a course.After one course,the clinical efficacy,safety and compliance of the three groups were analyzed and compared.Results: 1.Literature Study:(1)Acupoints analysis for relieving exterior:Frequency analysis shows that the frequency from high to low is Ershanmen,Nei Laogong,Sanguan,Xinjing,Jianjing,Taiyang,etc.Distribution analysis shows that the commonly used parts are hands,head and face,forearm and so on.Cluster analysis shows that the strongest clustering relationship is Taiyang and Fengchi.Correlation analysis showed that the high-frequency acupoints pairs are Nei Laogong?Xinjing,Sanguan?Xinjing,Xinjing?Nei Laogong,while the high-frequency acupoints combinations were Nei Laogong,Xinjing ? Sanguan.Centrality analysis showed that the acupoint with the highest centrality is Sanguan.(2)Acupoints analysis for anti-tussive: The acupoints from high frequency to low is Feijing,Li to Qian,etc.The commonly used parts are hands.The strongest clustering group is Erma and Ershanmen,Xiaohengwen and Wujing.The high-frequency acupoints pairs are Li to Qian? Feijing,Nei Bagua ? Feijing,while the high-frequency acupoints combinations are Nei Bagua and Feijing?Pijing,Pijing and Feijing?Nei Bagua.The acupoint with the highest centrality is Feijing.2.Clinical Study:(1)Overall efficacy evaluation: The markedly effective rate of treatment group(81.82%)is significantly higher than that of control group 1(50.00%)and group 2(47.06%)(P<0.05).(2)Comprehensive scores of symptoms and signs:The comprehensive scores of treatment group and control group 1 are significantly different from group 2(P<0.05).(3)Comprehensive scores of main symptoms: There is significant difference of main symptoms comprehensive scores between treatment group,control group 1 and group 2(P<0.05).(4)Cough scores: There is significant difference between the treatment group and control group 2(P<0.05).(5)The disappearance time of dry and wet rales in lung: The disappearance time in treatment group and control group 1 is significantly different from group 2(P<0.05).(6)The disappearance time of cough: The disappearance time of treatment group is significantly different from control group 2(P<0.05),but there is no significant difference between the treatment group and the control group 1(P>0.05).Conclusions: Based on the acupoints selection rules of treatment for pediatric wind-heat cough by Tuina in ancient and modern times,a new prescription is reconstructed.The application of this prescription has a good effect on wind-heat cough,which is better than that of the control group,which provides a new approach for pediatric cough and is worthy of clinical application.
