Objective:To explore the effect of acupuncture intervention on postmenopausal osteoporosis,further establish the relationship between body weight,intestinal flora and osteoporosis,and reveal partial mechanism of acupuncture in the prevention and treatment of postmenopausal osteoporosis on the basis of analyzing the relationship between body weight and osteoporosis.Methods:1.Clinical study:The bone mineral density(BMD)and related factors of healthy people who underwent physical examination in the Physical Examination Center of Jiangsu Province Geriatric Hospital from October 1 to December 31,2020 were analyzed.The BMD was measured by Osteosys ultrasound bone densometer(Korea,SONOST-3000),and the BMD of the left calcaneus was measured uniformly.By using the self-developed questionnaire on related factors,the doctors of the physical examination center distributed questionnaires to the physical examination personnel,and a total of 550 persons were screened out who met the inclusion criteria of the survey.According to gender classification,univariate multinomial logistic regression and multivariate multinomial logistic regression were used to analyze the bone health status of males and females as well as the related factors affecting the occurrence of osteoporosis(OP).The bone health status of the population and the related factors affecting the occurrence of OP in the above population were understood,and the correlation between BMD and body mass index(BMI)and visceral adipose index(VAI)was analyzed.2.Experimental study:3-month-old female SD rats were randomly divided into blank group,sham operation group,model group and electroacupuncture group,with 8 rats in each group.Rats in the model group and the electroacupuncture group underwent bilateral ovariectomy.The electroacupuncture group was alternately given "Guanyuan(CV4)+Sanyinjiao(SP6)"(double)and "Shenshu(BL23)+Housanli(ST36)"(double)electroacupuncture intervention for 20 min each time,once a day,for 5 consecutive days every week for 12 weeks.After 12 weeks,BMD of right tibia and femur was measured by bone densitometer.The body weight of rats in each group was weighed before and after the experiment,and the feces were taken for 16Sr DNA flora detection,and the short-chain fatty acids in the feces of rats were detected by gas chromatography.The short-chain fatty acids detected in this experiment were butyric acid,acetic acid,propionic acid,isobutyric acid,isovaleric acid,valeric acid.SPSS 25.0 was used for statistical processing of results.Results:1.Clinical research(1)Among the 550 people aged 30-80 in the Physical Examination Center of Jiangsu Province Geriatric Hospital,the number of people with normal bone mass screened out accounted for 32.9%,the number of people with osteopenia accounted for 50.2%,and the number of people with osteoporosis accounted for 16.9%.Among them,males with osteopenia accounted for 45.6%of the total number of males,and males with osteoporosis accounted for 10.8%;females with osteopenia accounted for 52.7%of the total number of females,and females with osteoporosis accounted for 20.3%.(2)The independent risk factors of male BMD were age and VAI(P<0.05,OR>1),and high BMI was a protective factor(P<0.05,OR<1).Age,menopause and VAI were independent risk factors of female BMD(P<0.05,OR>1),while high BMI and drinking milk were protective factors(P<0.05,OR<1).2.Experimental study(1)Changes of body weight in rats of each group:After 12 weeks,compared with the blank group,the body weight in the model group decreased significantly(P<0.01);compared with the model group,the body weight in the electroacupuncture group increased significantly(P<0.01);compared with the blank group and the sham operation group,the weight growth rate in the model group decreased significantly(P<0.05);compared with the model group,the weight growth rate in the electroacupuncture group increased significantly(P<0.01);compared with the blank group,BMI in the model group was lower(P<0.05),and compared with the model group,BMI in the electroacupuncture group was higher(P<0.01).(2)Changes of BMD in rats of each group:Compared with the blank group and the sham operation group,BMD of the model group was significantly reduced(P<0.05);compared with the model group,BMD in the electroacupuncture group was significantly increased(P<0.05);(3)Changes of intestinal flora in each group:At the phylum level,the abundance of Tenericutes,Actinobacteria and Deferribacteria in the model group increased significantly compared with the blank group and sham operation group(P<0.05),while the abundance of Firmicutes,Bacteroidetes,Proteobacteria and Melainabacteria decreased(P<0.05).Compared with the model group,the abundance of Firmicutes,Bacteroidetes,Proteobacteria and Melainabacteria in the electroacupuncture group increased(P<0.05),and the abundance of Tenericutes,Actinobacteria and Deferribacteria in the electroacupuncture group decreased(P<0.05).At the genus level,the abundance of Bacteroides,Lactobacillus and Enterococcus in the model group decreased(P<0.05).Compared with the model group,the abundance of Bacteroides,Lactobacillus and Enterococcus in the electroacupuncture group increased compared with the blank group and the sham operation group.(4)Changes of short-chain fatty acids in rats of each group:Compared with the blank group and the sham operation group,the contents of butyric acid,propionic acid,isobutyric acid and valeric acid in the model group were significantly reduced(P<0.05),and those in the electroacupuncture group was significantly higher than those in the model group.The content of butyric acid and propionic acid increased(P<0.05).Conclusion:Different obesity types have different effects on OP.High BMI has protective effect on BMD,and high VAI is an independent risk factor for BMD.The high BMI of rats in electroacupuncture group has protective effect on BMD,and electroacupuncture intervention can regulate the intestinal flora structure and short-chain fatty acid content of postmenopausal osteoporosis model rats.Acupuncture may reduce visceral fat and protect bone by regulating intestinal flora and short-chain fatty acids under the condition of high BMI.
