| Objective: To observe the clinical efficacy of auricular acupoint comprehensive therapy in the treatment of migraine without aura(MWo A).And to explore the influence of auricular acupoint comprehensive therapy on the coagulation mechanism of patients,and to provide evidence for the central mechanism of auricular acupoint comprehensive therapy from the perspective of functional imaging,which all based on TEG and rs-f MRI.it also provides a scientific basis for improving the diagnosis,treatment and prevention of auricular acupoint comprehensive therapy for MWo A.Methods:1.80 patients were randomized and controlled divided into two groups of auricular acupoint comprehensive therapy group(treatment group)and the auricular acupuncture treatment group(control group),after one course treatment,changes of VAS,SDS,SAS,WHOQOL-BREF score and migraine score were observed and analyzed.2.The TEG was examined and the changes of parameters was observed in the two groups of patients before and after treatment.3.To observe the changes of Re Ho and ALFF scores in the patients' brain areas and to search for abnormal brain areas,patients in the auricular acupoint comprehensive therapy group were given one rs-f MRI examination before and after treatment.Results:1.VAS,SDS score and migraine score in both groups were significantly reduced after treatment,and WHOQOL-BREF score was significantly increased.There was no significant difference in VAS,SDS score and migraine score between the two groups(P>0.05).There was a significant difference in WHOQOL-BREF score(P < 0.05),and the improvement was more obvious in the observation group than in the control group.The total effective rate of the observation group was higher than that of the control group,with a significant difference(P < 0.05).2.In terms of TEG,K score,Angle score,MA score,CI score,A score and G score were significantly changed in the observation group after treatment(P<0.05),while no significant changes were observed in the control group(P>0.05).Compared with the two groups,K score,Angle score,MA score,A score,G score and CI score in the observation group after treatment had significant changes compared with the control group(P<0.05),while there was no significant changes in R score,LY30 score,EPL score and PMA score(P>0.05).3.The score of Re Ho of the patients increased in the right temporal middle gyrus,right middle frontal gyrus,bilaterally in posterior central gyrus.The score of Re Ho of right hippocampal gyrus,right inferior temporal gyrus,right caudate nucleus,bilateral cerebellum,left middle temporal gyrus,left thalamus,left precuneus,left medial and paracentulate gyrus decreased.The score of ALFF of left cerebellum,middle temporal gyrus,orbital middle frontal gyrus,superior temporal gyrus,angular gyrus and posterior central gyrus increased.Conclusion:1.Auricular acupoint comprehensive therapy is effective in the treatment of migraine without aura.It can effectively improve the pain degree,frequency,duration,concomitant symptoms and depression,and improve their quality of life.2.According to the results of TEG,auricular acupoint comprehensive therapy can improve the coagulation function of patients by interfering with the coagulation mechanism to inhibit the function of fibrinogen and the function of thrombocytopenesis,and then block the occurrence and development of migraine.3.The results of rs-MRI suggest that auricular acupoint comprehensive therapy can improve the function of related brain areas and brain functional networks by affecting the synchrony and activity intensity of local neurons in the brain region,and then intervene the perception,transmission,processing in pain information,and emotional processing,cognitive adjustment and other processes. |
