With the improvement of modernization and the change of life style in our country, trauma incidences have increased greatly, which has heavily threatened our health and quality of life as a nosogenetic factor. Therefore more and more attention has been paid to the treatment of maxillofacial trauma. Because of their special anatomical and physiological characteristics in the oral and maxillofacial region, not only maxillofacial injuries are frequently found in multiply injured patients, but also maxillofacial injury may cause permanent impediment of functions and destroy of facial figure, which is different from injuries in other regions. So it is an important study for maxillofacial trauma.Now maxillofacial trauma severity scores has not uniform criterion in domestic and foreign country, the epidemiological study of maxillofacial trauma has not uniform criterion either, which makes the outcome of epidemiological study have no scientific research value. Therefore more and more attention has been paid to the treatment of maxillofacial trauma. Moreover the existing traumatic appraisal methods can not evaluate maxillofacial trauma availably, so it is necessary to study a maxillofacial traumatic appraisal methods accorded with maxillofacial surgery characters. Maxillofacial trauma severity scores had been reported rarely in foreign papers, but there were many research papers of this problem in our county. But they did not realize that maxillofacial anatomic trauma can affect maxillofacial function, their appraisal methods mixed maxillofacial anatomic trauma and maxillofacial function, and trauma indexes were too simple and incomplete. Thus their methods can't show exactly maxillofacial injury severity. The purpose of this study is to establish a method of trauma severity scores evaluation, which makes different trauma can be compared, provides uniform criterion for epidemiological research and trauma database.Overall consideration is required for the maxillofacial trauma treatment, as of the procedure of these which are included by rescue at the secene of the trauma, prehospital rescue, maxillofacial management at the hospital, it is necessary that the management should be consecutive and timely. So it is higher required for the procedure of maxillofacial trauma rescue. Although the level of maxillofacial trauma treatment is very high for maxillofacial specialist, but the procedure of maxillofacial trauma rescue is accomplished by the no-special paramedic before special treatment. Because of their limited special level, there has a huge gap in the rescue. So the treatment of mal-healed and obsolete fractures, bone no-union, infection, function impediment, secondary deformaities are very difficult. It will bring forward to puzzle for seamless nature of treatment. In order to increase the level of treatment and rescue for maxillofacial trauma, expert system is developed by using nerve network technique and classical model in our task. It has characteristic of canonical and standardized score system and can provide scheme of diagnosis and treat.The foundation of expert system only can be true diagnosis and grade of maxillofacial trauma to one patient. If outcome of diagnosis and treatment of some maxillofacial trauma patients will be registered, we have to develop a powerful database. Otherwise it will lead to loss of a large amount of maxillofacial trauma data, and make it very difficult to study maxillofacial trauma included information inquiry, epidemiological survey and other.By establishment of maxillofacial trauma database, not only record of data is ruled, but also clinical data is effectually stored and shared. It will be in favor of develop collaboration of scientific research and enhance validity of maxillofacial trauma prevention and cure and increase reliability and scientificity of epidemiological study. Maxillofacial trauma database is established by Microsoft Access 2003 software in our task, which have some functions included diagnose, grade, treat, inquire, store, and et al.Major experimental results from the studies are:1. We have established and perfected professional maxillofacial injury severity score, which continue to consult some experts with maxillfacial injuried items and their classifications based on our master's task. Every injury item in the same hierarchy was valuated by other items through Delphi technique, to gain compounded weight of every item. The results showed that the appraisal means of maxillofacial anatomic and functional injury severity was obtained, the appraisal methods was expressed by the sum, which was added by the product of the average compounded weigh and classification of appraisal items severity degree. Two mathematics models were established which were injury severity score of maxillofacial anatomy and function. Then 60 clinical maxillofacial traumatic cases were selected, anatomic and functional injury severity were summed respectively, the correlation of two sums models were analyzed. Mathematics model of injury severity scores have statistics significant correlation between the anatomic and functional injury, Correlative coefficient ( 0.431 ) was obtained, correlation was significant at the 0.01 level. It demonstrated that maxillofacial injury items and their classifications were optimized, made them reasonable and exacte.2. Every injury comprises of some clinical manifestations, in which can most confirm clinical diagnosis of injury that it is principle of weight maximum eigenvalue. In our study, weight of injuried clinical manifestation was applied on the basis of professional information, in order to provide mathematics operational data for exploitation of expert system. In combination with weight of maxillofacial injury item and its classification, figured out the corresponding weight of every injury degree, and worked up plan of treatment which was based on injury degree. In order to code when expert system is developed.3. Microsoft Visual C++ 6.0 was employed to develop expert system, function of expert system was accomplished which included of diagnose diease, trauma score, plan of treatment, et al. this system had clear interface, easy control. System has satisfactory stability, security. Diagnosis is accurate, and manipulation is convenient. It can be used as an auxiliary tool of beginner and teaching, and also may supply plan for pre-hospital diagnosis and treatment.4. Based on relational database structure, we have developed maxillofacial trauma database using Microsoft Access 2003 software, and expert system was embedded in database, which has professional characteristic of maxillofacial trauma. This system have some functions which consist of diagnose, grade, treat, inquire, store.
