A Conversation Study Of Outpatient Doctor-Patient Based On Communicative Intention

Doctor-patient communication is a comprehensive research field that integrates sociology,psychology,linguistics,institutional science and medicine.In the seventies of last century,the doctor-patient communication was officially established as a big research field.In our country since 1990 s,with the putting forward of concept of harmonious society,the doctor-patient relationship is not only a subject that needs to be studied in the field of medicine,but also involves many fields of multidisciplinary research.Research on strengthening the theory and practice of doctor-patient conversation can effectively ease the tension between doctors and patients,improve doctor-patient relationship,increase doctor-patient communication and increase medical efficiency.This article will focus on the doctor-patient conversation in the outpatient clinic and analyze the communicative intention of the doctor-patient conversation.Different from the previous studies on the existing texts that have been generated,this paper attempts to start from the initial motivation of doctor-patient conversation to analyze the intent of verbal communication activated by motivation.When a patient arrives at a hospital for medical treatment,he creates the need of how to solve his own illness.Under this kind of need stimulation,the intention of the patient to communicate with the doctor arises so as to be satisfied.This is the most fundamental analysis of the origins of doctors and patients.Then proceed from the source,this article shall classify the communicative intention of doctor-patient conversation.This article shall be divided into seven parts:Introduction.It summarizes the previous scholars’ analysis of doctor-patient conversation from different perspectives,and find out blank.The blank is lacking of analysis of doctor-patient conversations in outpatient service from communicative intention,that is research perspective of this paper.On this basis,the research significance,research object,research ideas,linguistic data source are introduced.In research perspective,this paper summarizes the significance of the research of doctor-patient communication activities from the perspective of theory and practice.Inresearch object,the main communicative intention of this paper is demanding communicative intention and informed communication intention.In terms of thinking,this paper analyzes demanding communicative intention and informed communication intention of doctor-patient in frame of institutional discourse.On that basis,analyze realization of communicative intention in special discourse form.This corpus collects a complete audio-visual recording of outpatient conversations between 22 young,middle and-senior-middle-aged male and female doctors at two top three hospitals in one city and 100 young,middle-aged and young patients(and their relatives).Chapter one,the literatures review.This chapter combs scholars’ research on doctor-patient conversation.It summarizes the previous scholars’ analysis of doctor-patient conversation from different perspectives,including linguistics-related theoretical perspectives,linguistic theory and medical practice,and humanities and social studies.The advantages and disadvantages of each angle are analyzed,and we define the purpose of this thesis is study of the communicative intention in doctor-patient communicative activities.Secondly,it summarizes correlational research of communicative intention succinctly.Chapter two,The characteristics and structure of verbal communication between doctors and patients.Doctor-patient verbal communication is a special kind of communicative activity.It has the characteristics of general verbal communication,such as sociality,interactivity,purpose and behavior.At the same time,the doctor-patient communication is belonged to an institutional discourse.Therefore,on the basis of summarizing the characteristics of institutional discourse,the institutional nature of doctor-patient communication is analyzed.Doctor-patient communication is limited by institutions“unequal”,such as the position of the two communicating parties,information resources,medical resources and so on,so the doctor-patient communication belong to vertical communication In addition,this chapter analyzes the structure of doctor-patient verbal communication in the aspects of communicative subjects,communicative intention,communicative form,communicative environment and communicative purpose.Chapter three and Chapter four will analyze clinical doctor-patient conversation from demanding i behavior and notifying behavior from the macroscopic aspect.Chapter three,doctor-patient demanding informative behavior.First of all,this chapter analyzes the realization conditions and the realization structure of the doctor-patient informative intention,then analyzes the types and characteristics of the demanding informative intention from the doctor and the patient respectively based on the corpus data.Second,in the demanding informative intention,both the doctor and the patient have implemented the request-fulfillment and the request-to-communicate informative intention.Chapter four,doctor-patient informed verbal communication behavior.First of all,this chapter also first analyzes the implementation conditions and implementation structure of doctor-patient communication intention,and then analyzes the types and characteristics of notifying informative intention from the doctor and the patient respectively based on corpus data.The notifying informative intention of the doctors is divided into illness,examination,therapeutic and comforting notifying.Because the notifying informative intention of the patients are concentrated in the diseased part,they are analyzed both proactively and passively.Chapter five and Chapter six analyze the communicative intents in doctor-patient conversation activated by special form of words from the microscopic point of view,which are repetitive words and informative intention activated by "I / you said(see)".Chapter five,informative intention activated by repetitive words in clinical doctor-patient conversation.The informative intention is activated by the discourse form,and the repeated discourse form can also activate different informative intention.After classifying the corpus,the types of the informative intention to activate are divided into three types: seeking confirmation,obtaining information,and correcting information.Among them,information correction includes self-correction and modification of others.At the same time,combined with corpus data,this chapter analyses the characteristics of doctors and patients in the use of repetitive words.Chapter six,the informative intention activated by useing "I / you said(see)" in the doctor-patient conversation.This chapter first defines the scope ofthe subjective phrase "I / you said(see)" that is used in the doctor-patient conversation.Second,using the spss software in conjunction with the data,this chapter respectively analyses the informative intention of doctors and the patients activated by using the discourse form "I see,I say,you see / said" and the existence of the interaction between such.Chapter seven,conclusion.This chapter discusses the research prospects and the shortcomings of this article.
