The Clinical Effect Of Baihe Recipe In The Treatment Of Advanced Gastric Cancer And The Experimental Study Of Its Mechanism

Objective:To observe the clinical effects of the Baihe Recipe combined with chemotherapy in the treatment of advanced gastric cancer and the experimental study of its mechanism.Methods: 1.Clinical trialsEighty-three cases of advanced gastric cancer were randomly divided into two groups: the treatment group(40cases)and the control group(43cases).The treatment group patients were treated by Baihe Recipe combined with chemotherapy.The control group patients were treated by chemotherapy lonely.The difference between two groups of patients in tumor lesion?TCM symptom effect?quality of life(Karnofsky score)?QOL and seroma tumor marks were compared and analyzed.2.Experimental researchSixty nude mice orthotopically transplanted with BGC-823 human gastric cancer cells were randomly divided into four groups: untreated control group with isotonic saline 0.5 ml/d taken orally ?5-fluorouracil(5-FU)-treated group was given 5-FU dilution 0.20ml(60mg/kg per week,intraperitoneally injected)?Baihe Recipe-treated group with Baihe Recipe 0.5ml/d taken orally and Baihe Recipe combined with 5-FU group(Baihe Recipe 0.5ml/d taken orally and 5-FU dilution 0.20ml/week intraperitoneally injected).The mice were sacrificed and the growth and metastasis of tumor were observed.The weights of tumor were measured and some tissues of tumor were taken to produce electron microscopy specimens.The tumor MVD and the expressions of MMP-9?VEGF and P53 were detected by immunohistochemistry.Results: 1.The clinical research shows that: the stability rate of solid tumor in the treatment group and the control group were 75.00% and 53.49%,respectively with significant difference(P<0.05);the total effective rate of TCM syndromes in the treatment group and the control group were 77.50% and 58.13%,respectively with no significant difference(P>0.05);Karnofsky score of the treatment group increased significantly compared with the control group after treatment with a significant difference(P<0.05);the total effective rate of quality of life score in the treatment group and the control group were 72.5% and 53.48%,respectively with no significant difference(P>0.05);the serum levels of CEA and CA19-9 in the two groups were significantly decreased after treatment,and the reduction rate in the treatment group was higher than that in the control group,but with no significant difference(P>0.05).2.The experimental research shows that: the incidence rates of metastasis in perigastric lymph notes?lymph notes in porta hepatis ? liver and peritoneum in the 5-FU treated ?Baihe Recipe treated and Baihe Recipe combined with 5-FU treated groups were lower than those in the untreated group;the total metastasis rates for each groups were 40.00%?40.00% ?35.71% and 80.00%,respectively with significant difference(P<0.05);The growth-inhibiting rates in the 5-FU treated groups? Baihe Recipe treated groups and Baihe Recipe combined with 5-FU treated groups were 48.65% ?47.64% and54.39% respectively and tumor weight were significantly lower than that in the untreated group(P<0.05).Immunohistochemistry showed that tumor MVD in the Baihe Recipe treated group and Baihe Recipe combined with 5-FU treated groups were significantly lower than those in the 5-FU treated groups and untreated group,the expressions of VEGF and P53 in the Baihe Recipe treated groups and Baihe Recipe combined with 5-FU treated groups were lower than those in the untreated group and 5-FU treated group with a significant difference(P<0.05),even the expressions of MMP-9 in the Baihe Recipe treated groups and Baihe Recipe combined with 5-FU treated groups were lower than those in the untreated group with a significant difference(P<0.05).Conclusions: 1.Baihe Recipe combined with chemotherapy can improve the quality of life ?tumor stability and physical condition of patients with advanced gastric cancer.Baihe Recipe can improve the short-term efficacy.2.Baihe Recipe can inhibit the growth and metastasis of human gastric carcinoma orthotopically transplanted in nude mice and reduce the MVD of the transplanted tumor tissue by reducing the expressions of MMP-9?VEGF and P53.It might be one of the mechanisms responsible for its anti-metastasis effects.
