From birth to the age of three,the primary teeth begin to erupt and decay can occur shortly after the primary teeth eruption.The development of primary teeth has been started since the embryonic period.Primary dentition begin to erupt at 6 months after birth,and is fully formed at the age of three.The development and eruption of primary teeth are affected by various factors,such as maternal,perinatal,and postnatal factors.Caries is one of the most common forms of oral diseases.Early childhood caries is a special form of caries,it is one of the main diseases affecting children's health.The research on the etiology of early childhood caries is of great significance to the health maintenance of the children.Cohort study is the study of high reliability and the birth cohort is the typical ways,with high etiology evidence and other incomparable advantages.Therefore,it is of great significance to carry out birth cohort in oral medical research for the etiology and prevention of oral diseases.The oral health birth cohort of Nanjing maternal and child health care hospital was composed of 1,296 pregnant women and their children who were enrolled in nanjing maternal and child health care hospital from May 2014 to September 2015.From baseline data and biological samples collected during pregnancy and birth,infants aged one and three were invited to return for clinical examinations and biological sample collection.This project will be conducted from the following two parts:Part ?.The influence factors of primary teeth eruption timing in one year old children in NanjingAims The aim of our study was to investigate the associations of maternal,perinatal and postnatal factors with the first primary tooth eruption timing in Nanjing population.Methods We recruited 1296 pregnant women and their children from the Nanjing maternal and child health care hospital.Maternal demographic information and clinical data during prenatal and perinatal period were collected,and oral examination of infants were performed 12 months of age.Multiple regression analysis was used to identify significant explanatory variables for the eruption timing of the first primary tooth.Results A total of 1109 children were included in the result analysis.The mean first primary tooth of all infants was 6.82 ± 1.90 months.The first two eruption peaks for both male and female infants were in 6 and 8 months.During the first 9 months of life,89.57% of male infants had their first primary tooth eruption,but 86.83% of female infants.After adjustment for confounders,higher maternal childbearing age(? = 0.56,95%CI = 0.11-1.01),female infants(? = 0.28,95%CI = 0.05-0.50),and low birth weight(? = 0.94;95% CI = 0.15-1.74)were significantly associated with delayed eruption of first primary tooth,while high birth weight(? =-0.85;95% CI-1.36--0.33)was significantly associated with earlier eruption of first primary tooth.Conclusion Infant sex,maternal childbearing age and infant birth weight are significant determinants of the first primary tooth eruption time.Part ?.The influence factors of caries in three years old children in NanjingAims To investigate and analyze the relationship between maternal,perinatal,postnatal factors and the dental caries of three years old children in Nanjing population.Methods The basic population was same to the first part,the clinical follow-up visit of three years old begin September 2017 to February 2018,and the number of children returning to visit was 238.Among them,the questionnaire survey and clinical data during pregnancy,clinical data at birth,clinical oral examination data and questionnaire survey data of one year old have been obtained in the first part.Collected oral questionnaire survey and clinical examination at three years old.Statistical analysis was performed.Results At the age of one,the primary teeth caries prevalence rate and caries mean were 2.94% and 0.08 and at the age of three,primary teeth caries prevalence rate and caries mean were 35.29% and 1.40 respectively.During the period of age one to three years,primary teeth caries incidence was 32.35%.The occurrence of caries were symmetrical.Primary tooth caries were often examined on maxillary deciduous incisor and the mandibular deciduous molar.After adjustment for confounders,higher maternal gestational weight gain(95%CI=1.022-1.159,P=0.008),higher birth weight(95%CI=0.998-1.000,P=0.008),and often eat sweet food(95%CI=0.195-0.740,P=0.004)were significantly associated with primary tooth caries.Conclusion Prevention of primary tooth caries should be carried out as early as possible.Higher maternal gestational weight gain,higher birth weight,and like eating sweet food were significantly associated with primary tooth caries.
