PURPOSEFemoral intercondylar notch stenosis is an anatomical risk factor of ACL injury.The traditional assessment parameters are two-dimensional parameters,including intercondylar notch width,intercondylar notch width index,and intercondylar notch shape.The intercondylar notch volume as a new three-dimensional evaluation parametr,which only a few researches refer to it,and lack of researches based on Chinese race.The purpose of this study is to explore the correlation between the intercondylar notch size and the ACL injury in the Han Chinese population on the basis of intercondylar notch volume,to analyze the effect of demographic factors on the intercondylar notch volume,to explore the relationship between intercondylar notch volume and intercondylar notch width and width index,and to propose a new method of evaluation for intercondylar notch,which provides a reference for the risk assessment of ACL injury.METHODSPART 1:Randomly selected 280 patients diagnosed and treated in the Sixth People’s Hospital of Shanghai from January 2015 to June 2016,including 140 ACL-injured patients(70 males and 70 females)and 140ACL-intact patients(70 males and 70 females).The TView software was used for the intercondylar notch volume measurement,and the SPSS software was used for statistics.The relationship between the intercondylar notch volume and ACL injury and the correlation between demographic factors and intercondylar notch volume were analyzed.PART 2:randomly selected 240 patients diagnosed and treated in the Sixth People’s Hospital of Shanghai from June 2015 to June 2016,including120 ACL-injured patients(60 males and 60 females)and 120 ACL-intact patients(60 males and 60 females).The intercondylar notch volume,width and width index were measured.The intercondylar notch was divided into four types,including no obvious stenosis,inlet stenosis,outlet stenosis and both inlet and outlet stenosis.The correlation between the two-dimensional parameters and the intercondylar notch volume and the correlation between the four types of intercondylar notch and the intercondylar notch volume,the risk of ACL injury were analyzed.RESULTSPART 1:The intercondylar notch volume of the ACL injury group was significantly smaller than that of the control group(5.94±1.35 cm~3 vs 6.86±1.61 cm~3,P<0.001).The female intercondylar notch volume was usually smaller than that of the male(5.53±0.93 cm~3 vs 7.28±1.55 cm~3,P<0.001).The intercondylar notch volume was moderately related to the height and weight(R=0.561,0.483),and was slightly correlated with BMI(R=0.281),but had no significant correlation with the age.PART 2:The intercondylar notch volume was moderately related to the intercondylar notch width at the sites of the popliteal groove,the inlet and ACL attachment(R=0.453,0.407,0.440),and low related to the width at the outlet(R=0.305).The intercondylar notch volume was not related to the intercondylar notch width index of the above four sites.The type of the both inlet and outlet stenosis intercondylar notch was associated with a smaller notch volume(P=0.03)and a higher risk of ACL injury(P=0.002).There was no significant difference in morphological distribution.between males and femalesCONCLUSIONFor the Chinese Han population,the intercondylar notch volume is smaller in the ACL-injured group than the ACL-intact group;females tend to have smaller intercondylar notch volume than the males;there are the moderate or low correlation between intercondylar notch volume and individual height and weight,and the low or no correlation with individual BMI,and no correlation with the age.The correlation between the intercondylar notch volume and the intercondylar notch width and width index are poor,and it is prudent to use the two-dimensional parameters to evaluate the three-dimensional intercondylar notch morphology.There was no significant difference in the morphology of intercondylar notch between males and females.The ones with the both inlet and outlet stenosis notch are tend to have the smaller notches and higher ACL injury risk,which is indicated to take notchplasty.
