Clinical And Experimental Study Of JianPiMoji Soup In Treating Hepatocellular Carcinoma

Objective:1.Clinical partIn patients with primary liver cancer who meet the inclusion criteria,The treatment group was treated with Jianpi Moji soup combined with percutaneous ethanol injection(PEI),and the control group was treated with PEI alone.Observing the clinical efficacy of Jianpi Moji soup in the treatment of advanced primary liver cancer.Explore its role in the comprehensive treatment of advanced primary liver cancer,so as to provide a new treatment plan for clinical treatment of primary liver cancer.2.Experimental partThe effect of Jianpi Grind Decoction on apoptosis,migration and invasion of human hepatocellular carcinoma HepG2 cells was investigated by experimental studies.Explain the possible mechanism of Jianpi Moji soup in treating primary liver cancer and provide theoretical basis for the clinical application of Jianpi Moji soup in treating primary liver cancer.Methods:1.Clinical studies: 50 patients with hepatocellular carcinoma who met the criteria of case selection from October 2016 to October 2017 were randomly divided into treatment group and control group.The treatment group was treated with Jianpi Grinding Decoction and Percutaneous Ethanol Injection(PEI).The control group was treated with PEI alone.The recent clinical syndrome scores,Kamofsky score,weight change assessment,RECIST solid tumor efficacy assessment,liver function(ALB,ALT,AST,TB),and renal function index(BUN,Cr)were used as observation indexes.Both groups were compared after 6 weeks of treatment to evaluate the short-term efficacy of Jianpi Moji soup in treating patients with primary liver cancer.2.Experimental study: Human hepatocellular carcinoma HepG2 cells were divided into control group,low-,medium-,and high-dose groups of Jianpi Grinding Decoction.MTT assay was used to observe the effect of Jianpi Grind Decoction on the growth of HepG2 cells.Flow cytometry was used to detect the effect of Jianpi Grinding Decoction on the apoptosis of human hepatocellular carcinoma cell HepG2.The effects of Jianpi Grind decoction on the migration and invasion of HepG2 cells were observed by scratch test and transwell chamber experiments.The effects of Jianpi Grind Decoction on the expression of P53,Bcl-2,and Bax proteins in HepG2 cells were detected by Western Blot assay and immunofluorescence assay.Results:1.Clinical study:1.Clinical research:(1)Comparison of the efficacy of recent tumors: The effective rate(RR)of th e treatment group was 16.00%,the clinical benefit rate(CBR)was 96.00%,the con trol group's effective rate(RR)was 12.00%,and the clinical benefit rate(CBR)wa s 84.00%,The efficacy of the two groups of tumors was compared by statistical ana lysis,the difference was not statistically significant(P>0.05)(2)Comparison of TCM syndrome scores:(1)Within 1 group: TCM symptom sc ores decreased after treatment in both groups,and both groups had statistical significa nce before and after treatment(P<0.05).(2)Comparison between the two groups: The s cores of TCM symptoms after treatment in the two groups were compared,and the scores in the treatment group decreased significantly.The difference was statistically s ignificant(P<0.01).(3)Comparison of TCM syndromes: The treatment group significantly improve d 5 cases(20.00%),partially improved 16 cases(64.00%),and did not improve 4 cases(16.00%),the improvement rate was 84.00%;There was no significant improve ment in the control group.Some improved cases were 10 cases(40.00%),15 cases(60.00%)were not improved,and the improvement rate was 40.00%.After statistica l analysis of the two groups,the improvement rate of TCM syndromes in the treat ment group was significantly better than that in the control group,and the differenc e was statistically significant(P<0.01).(4)Comparison of quality of life scores(card scores):(1)Within 1 group: KPS scores in the treatment group were significantly higher than those before treatment,and the difference was statistically significant(P<0.01).There was no significant dif ference between the control group and the pre-treatment group(P>0.05).(2)Compar ison between the two groups: KPS scores after treatment in the two groups were st atistically analyzed,and the difference was statistically significant(P<0.01).(5)KPS efficacy comparison: The treatment group improved in 13 cases(52.00%),stable in 11 cases(44.00%)and decreased in 1 case(4.00%);The control grou p improved in 4 cases(16.00%),stabilized in 16 cases(64.00%),and decreased in 5 cases(20.00%).After statistical analysis,the improvement rate of the quality of lif e of the treatment group was significantly better than that of the control group,and the difference was statistically significant(P<0.01).(6)Comparison of weight changes: In the treatment group,5 cases(20.00%)g ained weight,14(56.00%)were stable,6(24.00%)lost weight,and 2(8.00%)incr eased in the control group.Ten patients(40.00%)had stable body weight and 13 ca ses(52.00%)lost body weight.After statistical analysis,the treatment group had si gnificantly better weight gain than the control group.The difference was statistically significant(P<0.01).(7)Comparison of liver function: ALT,AST,and TB were decreased in the trea tment group before and after treatment(P<0.05);ALB,ALT,and AST were not sta tistically significant in the control group before and after treatment(P>0.05)..(8)Renal function: There was no significant change in BUN and Cr before an d after treatment in both groups(P>0.05).2.Experimental study:Jianpi Moji soup can inhibit the growth of HepG2,promote its apoptosis,inhi bit the migration and invasion of HepG2,down-regulate the expression of Bcl-2 pr otein and up-regulate the expression of Bax and P53 protein at the protein and gen e level,decrease the ratio of Bcl-2/Bax and the role of Jianpi Moji soup With dru g concentration-dependent.Conclusions:1.Jianpi Moji soup can significantly relieve clinical symptoms,improve the efficacy of TCM syndromes,and can significantly improve the quality of life of patients,can protect and restore liver function,and has a good safety,highlighting the Jianpi Moji soup for the treatment of liver cancer The advantages of the disease are worth further promotion in the clinic.2.Jianpi Moji soup has the effect of inducing apoptosis of HepG23.Jianpi Moji soup has the effect of inhibiting the invasion and migration of HepG24.the mechanism of Jianpi Moji soup inducing HepG2 apoptosis and inhibiting the invasion and migration of HepG2 human hepatoma cells may be related to down regulating the expression of Bcl-2,up regulating the expression of Bax and P53 protein and decreasing the ratio of Bcl-2/Bax.
