Research Of Anti-inflammatory Action Mechanism On Using Compound Zicao Oil For The External Treatment Of Pediatric Diaper Dermatitis

ObjectiveThis thesis aims at providing evidence of Compound Zicao Oil for the treatment of pediatric diaper dermatitis(DD)with experimental research,by researching anti-inflammatory action mechanism on using Compound Zicao Oil for the external treatment of pediatric DD.Methods1.UsingRevMan 5.3 software for Metaanalysis isto evaluate the clinical efficacy and safety of the treatment of pediatric DD by using Compound Zicao Oil for external use,To understand the effect o Compound Zicao Oil for the Treatment of Pediatric DD based on teh Dispersing Stagnated Fire Theory2.Since the pediatric DD tends to be allergic contact dermatitis(ACD),we used the young mice ACD models as pediatric DD models,and evaluated jointly the anti-nflammatory effect of external use of Compound Zicao Oil for the treatment of pediatric DDby the animal models of acute auricle swelling in young mice and of skin blood capillary permeability of hyperactivity in young mice as supplements.The research is to explore by HE staining to observe the pathological morphology of auricle in young mice,by nitrate reductase method detected the serum NO content in young mice,by ELISA to detect the protein content of IL-1?,TNF-? and IL-6,by immunohistochemistry to detect the expression of P2X7 receptor and NF-?B/p65 in auricle,and by Real-time RT-PCR to detect the expression levels of P2X7 receptor mRNA and NF-kappa B/p65 mRNA in auricle.Results1.The results of Meta analysis of the treatment of DD in children with external use of Compound Zicao Oil:There are 11 RCTS included in the study,with the total number of children DD1468.Uing Compound Zicao Oil for the external treatment of pediatric DD is better thanother local control treatments(external use rapeseed oil,sesame oil cream for hip,tannic acid ointment zinc oxide,erythromycin ointment or calamine lotion)in clinical efficiency,recovery rate and average recovery days,These differences are statistically significant(P<0.01),No adverse reactions are mentioned in 802 treatment groups of 11 RCT patients.2,To Understand Compound Zicao Oil for the Treatment of Pediatric DD based on Dispersing Stagnated Fire Theory:In Compound Zicao Oil,Zicao is the Monarch herb;Rendongteng and Baizhi are Minister herbs;Sesame oil is the Adjuvant herb;Bingpian is the guiding herb.Each herb has good anti-inflammatory effect,and can quickly repair the inflammatory skin,Treatment principles of the prescription are to clear away heat,cool the blood,remove toxic substances;and to dispel wind,resolve dampness,reduce the swelling;assisted with the herbs to moisten the skin The key to the treatment of Pediatric DD is to comply with the Dispersing Stagnated Fire Theory,and to clea,to cool,to resolve and to moisten and to dispel stagnated fire.3.Experiment1:The effect of external use of Compound Zicao Oil in ACD models in young mice.A series of experiments about the effect of external use of Compound Zicao Oil in ACD models in young mice,the result is:Compared to the model group,by external useof Compound Zicao Oil in ACD models in young mice the inhibitory effect on the degree of swelling of the auricle is considered statistically significant on ACD models in young mice(P<0,01),while the sesame oil is not considered statistically significant(P>0,05).By external use of Compound Zicao Oil it can obviously decrease the protein content of IL-1?,TNF-? and IL-6 in ACD young mouse serum(P<0.01),while teh sesame oil is not(P>0.05);it can obviously decrease the serum NO content in ACD young mouse(P<0.01),while the sesame oil is not(P>0.05).By HE staining,fluocinonide ointment group,Compound Zicao Oil high concentration of liquid group(2 times the concentration),Compound Zicao Oil group after external use,the auricle swelling,hyperemia and inflammatory cell infiltration are significantly improved under teh microscope.By immunohistochemistry,fluocinonide ointment group,Compound Zicao Oil high concentration of lnquid group,Compound Zicao Oil group after external use,the positive expression of P2X7 receptor and NF-?B/p65 is also significantly decreasedwith the significant improvement of tho inflammation,By Real-time PCR,the expression levels of P2X7 receptor mRNA is significantly higher that those in mormal group(P<0.01),and the expression levels of NF-kappa B/p65 mRNA are significantly higher(P<0.05).Comparde to the model group,the expression levels of P2X7 receptor mRNA is significantly decreased in the fluocinonide ointment group,the Compound Zicao Oil high concentration of liquid group,the Compond Zicao Oil group(P<0,01);the expression levels of NF-kappa B/p65 mRNA is also significantly decreased in the fluocinoni de ointment group,the Zicao Oil group,the Compound Zicao Oil high concentration of liquid group,the Compound Zicao Oil group(P<0.01),while the expression levels of P2X7 receptar mRNA in the sesame oil group,teh Zicao Oilgroup,the Compound Zicao Oil low concentratin of liquid group,and the expression levels of NF-kappa B/p65 mRNA in the sesame oil group,the Compound Zicao Oil low concentration of liquid group,all of them are lower than the model group,but is not significan(P>0.05).4.Experiment2:The effect of external use of Compound Zicao Oil in acute auricle swellingmodels in young mice.A series of experiments about the effect of external use of Compound Zicao Oil in acute auricle swelling models in young mice,the result is:Compared to the modol group,by preventive or therapeutic external use of Compound Zicao Oil,the inhibitory effecton the degree of swelling of the auricle is not considered statistically significant on the auricle swelling models in mice induced by dimethylbenzene(P>0.05).Compared to the model group,by preventive or therapeutic external use of Compound Zicao Oil,the inhibitory effect is considered statistically significant on the auricle swelling models in mice induced by crotonis fructus oil(P<0.01).By preventive or therapeutic external use of Compound Zicao Oil,it can obviously decreasethe protein content of TNF-? and IL-6 in auricle swelling young mouse serum(P<0,01);it can obviously decreasethe serum NO content in auricle swelling young mouse(P<0.01).By HE staining,after therapeutic external use of Compound Zicao Oil,the auricle swelling,hyperemia and inflammatory cell infiltration are significantly improved un der the microscope,By immunohistochemistry,after therapeutic external use of Compound Zicao Oil,the positive expression of P2X7 receptor was also significantly decreased with the significant improvement of the inflammation5.Experiment 3:The effect of external use of Compound Zicao Oil in skin blood capillary permeability of hyperactivity models in young mice:Compared to the model group,by xternal use of Compound Zicao Oil,the inhibitory effect is considered statistically significant on skin blood capillary permeability of hyperactivity models in young miceinduced by dimethylbenzene(P<0.05).While by external use of sesame oil,Zicao Oil or Compound Zicao Oil low concentration of liquid(1/2times the concentration),the inhibitory effect is not considered statistically significant(P>0.05).Conclusion1.According to The results of Me ta analysis of the treatment of DD in children with external aplication of Compound Zicao Oil,it is concluded that:Using Compound Zicao Oil for the local external treatment of pediatric DDcan obviously improve the clinical efficiency and recovery rate,shorten the treatment time greatly,and no adverse reactions are observed.2.To Understand the Comtpund Zicao Oil for the Treatment of Pediatric DD based on Dispersing Stagnated Fire Theory,it is concluded that:Compound Zicao Oil is suitable for children with DD,and all the drugs in the prescription have good anti-inflammatory effect.3.A series of experiments about the effect of external use Compound Zicao Oil in ACD models in young mice,it is concluded that:Eeternal use of the Compound Zicao Oil has therapeutic effect on ACDmodels in young mice.Its mechanism may be related to inhibit the NO content in serum,the protein content of IL-1?,TNF-? and IL-6 in serum,decreased the positive expression of P2 X7 receptor and NF-?B/p65in auricle tissue,decreased the expression levels of P2X7 receptor mRNA and NF-kappa B/p65 m R N Ain auricle tissue.It is possible to play an anti-inflammatory role by inhibiting the P2X7 receptor/NF-kappa B cell pathway.4.A series of experiments about the effect of external use of Compound Zicao Oil in acute auricle swelling models in young mice,it is concluded that:The external use of the Compound Zicao Oil has preventive and therapeutic effect on acute auricle inflammation in young mice.Its mechanism may be related to inhibit the NO content in serum,the protein content of TNF-? and IL-6 in serum,decrease d the positive expression of P2X7 receptor in auricle tissue.5.The effect of external use of Compound Zicao Oil in skin blood capillary permeability of hyperactivity models in young mice,it is concluded that:It has a good inhibitory effect for external use of Compound Zicao Oil in skin blood capillary permeability of hyperactivity models in young mice.Genral conclusions:External use of Compound Zicao oil for treatment of pediatric DDis good anti-inflammatory,which mechanism may be related to inhibition of the P2X7 receptor/NF-B cell pathway.
