A Clinical Analysis Of The Retinal Ganglion Cell-inner Plexiform Layer And Retinal Nerve Fiber Layer In Patients With Neuromyelitis Optica: Optical Coherence Tomography In Optic Neuritis

Aim:To utilize high-definition optical coherence tomography(HD-OCT)to determine the thicknesses of the retinal ganglion cell-inner plexiform layer(GCIPL)and retinal nerve fiber layer(RNFL),thickness of the neurosensory retina in patients with neuromyelitis optica(NMO)and to compare these measurements to those in normal eyes,providing a preliminary investigation into the use of OCT to detect optic nerve damage in patients with NMO.Methods:We conducted a cross-sectional study that included 30 NMO patients for a total of 60 eyes.According to the presence or absence of optic neuritis(ON),The subjects were divided intoNMO-ON group(30 eyes)and control group(10 eyes)NMO-ON.The control group was composed of 30 healthy persons(30 eyes)matched by age and sex.A detailed ophthalmologic examination was performed on both groups;then,the GCIPL and RNFL were measured using OCT.The GCIPL and RNFL thickness,thickness of the neurosensory retina measurements of all three groups were analyzed utilizing a one-way analysis of variance.Results:In the NMO-ON group,the mean GCIPL thickness was 60.00±11.70mm,the minimum GCIPL thickness was 51.23±14.32mm,the other 61.67±13.58 um,58.77±16.09 um,57.43±13.97 um,58.27±13.18um;60.30±13.19um;62.63±12.34 um in the superior,inferior,temporal and nasal quadrants etc respectively;and the thickness of the neurosensory retina is center thickness 232.63±28.03 um,volume 9.10±0.58mm~2,mean volumethickness255.43±16.15 um,N1282.83±21.82 um,N2284.80±20.35 um,N3278.57±17.01 um,N4 277.37±18.10 um,W1 252.23±46.29 um,W2 260.37±17.77 um,W3246.23±21.31 um,W4 247.57±18.48 um and the RNFL thicknessesweremean75.67±20.00 um,95.20±31.36 um,58.53±8.96 um,89.07±29.78 um,55.53±20.41 um in the superior,inferior,temporal and nasal quadrants respectively;The mean deviation is7.55±5.28 and corrected pattern standard deviation is 3.88±1.70?In the NMO-ON control group,the mean GCIPL thickness was 70.90±8.90mm,the minimum GCIPL thickness was 61.20±15.06mm,76.70±11.43mm,76.40±12.83mm,71.60±11.96mm,68.20±15.58mm,68.30±18.14mm,76.40±6.29mm in the superior,inferior,temporal and nasal quadrants,etc,respectively.the thickness of the neurosensory retina is228.60±18.06mm,9.62±0.62,267.30±16.61mm,N1 307.40±17.66mm,N2 300.30±29.91mm,N3 290.90±30.66mm,N4 293.30±10.76mm,W1 276.20±21.33mm,W2 285.40±23.25mm,W3 258.80±20.31mm,W4 258.50±19.59mm in the quadrants,etc,respectively;the RNFL thicknesses were 84.10±14.81mm,104.20±24.20mm,57.10±9.29mm,113.80±22.18mm,61.50±21.20mm in the mean,superior,inferior,temporal and nasal quadrants,respectively.The mean deviation(MD)is 2.60±2.25 and corrected pattern standard deviation is2.51±0.53?In the control group,the mean GCIPL thickness was 85.07±3.81mm,the minimum GCIPL thickness was 81.00±5.88mm,87.70±4.88mm,89.10±7.72mm,85.10±6.21mm,81.30±5.34mm,84.53±4.43mm,84.57±4.23mm in the superior,inferior,temporal and nasal quadrants,etc,respectively;the thickness of the neurosensory retina is 234.23±17.73mm,10.06±0.38mm~2,279.70±10.25mm,N1 318.70±16.80mm,N2 317.00±16.53mm,N3312.30±15.41mm,N4 303.00±15.0mm,W1 286.70±10.46mm,W2 303.50±20.31mm,W3261.20±48.55mm,W4 266.47±14.06mm in the quadrants,respectively;and the RNFL thicknesses were 115.63±12.03mm,131.53±17.45mm,69.90±13.23mm,129.10±15.84mm,87.40±18.43mm in the mean,superior,inferior,temporal and nasal quadrants,respectively.The GCIPL and RNFL thickness measurements in the NMO-ON control group were thinner than those in the control group(P<0.05);additionally,in the NMO-ON group,the mean,minimum GCIPL and the RNFL thickness of the inferior quadrant was lower than that in the control group(P<0.05).A significant correlation was seen between the GCIPL and RNFL thickness measurements and between the two visual field parameters,mean deviation and corrected pattern standard deviation,in the NMO-ON group(P<0.05).Conclusion:The thicknesses of the GCIPL and RNFL,measured by OCT,may indicate rentinal damage in patients with NMO.
