Background and ObjectiveAtrial fibrillation is one of the most common arrhythmias in clinical practice,accounting for about 1/3 of all arrhythmia inpatients.Among western industrialized countries,the prevalence rate is about 1-1.5%,In recent years,with the aging of population,the application of new technology and the enhancement of individual health care awareness,the incidence of atrial fibrillation is increasing.At present,the occurrence and maintenance of atrial fibrillation involves two aspects,the first is about trigger factor,the second is the matrix that occurs and sustains.With the development of electrophysiology and molecular biology,there are many important achievements in the pathogenesis of atrial fibrillation,especially the understanding of the reconstruction of atrial tissue.Atrial fibrillation can change the electrical and organizational characteristics of the atrium-Atrial reconstruction.The early stage of atrial remodeling showed that the ion channel function changes the electrical reconstruction,and the later stage is the tissue reconstruction with the characteristic of atrial fibrillation.atrial structural remodeling affect electrophysiological remodeling.In recent years,with the development of molecular biology,a series of studies have confirmed that TGF-beta 1 is the most important fiber growth factor.It is one of the important factors involved in myocardial reconstruction,TGF-beta 1 participates in various biological processes upstream,and regulates signal transduction pathways.It stimulates the proliferation of myocardial fibroblasts directly,It is a common pathway that causes myocardial fibrosis.with the development of clinical electrophysiology technology,it proves that the voltage amplitude of electrophysiological dissection can reflect the electrophysiological abnormality of myocardium.This abnormality of electrophysiological phenomena indirectly reflects the degree of myocardial fibrosis.There was no study on the correlation between TGF-beta 1 and atrial potential.Based on this,we proposed to compare the expression differences and related factors of plasma TGF-beta 1 in patients with common rapid arrhythmia in three groups.,and the correlation between TGF-1 and the average voltage of left atrium was analyzed by comparing the results of the first part,the aim is to better understand the pathophysiological mechanism of TGF-beta 1 in atrial fibrillation disease,so as to develop new treatment methods with high specificity and low side effects for TGF-beta 1 tachycardiaObjective:Detection and analysis and comparison of coronary sinus and the source of peripheral vein TGF-beta 1 level in the three groups of different type and group differences between a group of patients with cardiac arrhythmias,evaluation of different types of fast arrhythmia myocardial fibrosis in patients with active degreeMethods:Selected 96 underwent radiofrequency ablation surgical treatment of rapid arrhythmia patients,26 patients with PSVT patients and 44 patients with PAF,26 patients with CAF,respectively take coronary sinus and peripheral venous blood samples,using ELISA method to detect the plasma TGF-beta 1 level,the TGF-beta 1 original number after taking the numerical combined with clinical basic data for statistical analysisresults:In the three groups of patients with different arrhythmia types,the level of TGF-beta 1 in peripheral blood was higher than that of the coronary sinus TGF-beta 1.Conclusion:Compared with the three groups of samples,peripheral venous TGF-beta 1 was higher than that of the coronary sinus,indicating that TGF-beta 1 was expressed in multiple organs of the body.Part IIStudy on plasma TGF-beta 1 level and left atrial average voltage in patients with persistent atrial fibrillation.Objective:Detection the difference of TGF-beta 1 level between coronary sinus and peripheral vein,find the correlation between TGF-beta 1 level and left atrial average voltage,to assess the degree of myocardial fibrosis in patients with persistent atrial fibrillation indirectly.Methods:16 patients with persistent atrial fibrillation were treated with radiofrequency ablation.results Coronary sinus and peripheral venous blood samples were taken respectively,the plasma TGF-beta 1 level was detected by ELISA,The high density of electrophysiological dissection of left atrium was observed after radiofrequency ablation of sinus rhythm.The primary numerical value of TGF-1-1 was statistically analyzed with the clinical data.results:1)Patients with persistent atrial fibrillation(TGF-)1 level were higher than that of the coronary sinus TGF-beta 1;2)The level of TGF-beta 1 in the coronary sinus was negatively correlated with the average voltage of the left atrium,and there was a negative correlation between peripheral blood plasma TGF-beta 1 and the left atrial average voltage.Conclusion:1)Patients with persistent atrial fibrillation(TGF-beta 1)were higher than the coronary sinus,indicating that TGF-beta 1 was expressed in multiple organs of the body.2)There is a negative correlation between the level of TGF-beta 1 in the coronary sinus and the average voltage of the left atrium,and the level of TGF-beta 1 reflects the degree of atrial fibrillation.
