Part ?The clinical significance and prognostic value of 18F-FDG PET/CT in patients with non small cell lung cancerBackgroundThe incidence and mortality of lung cancer in the world on the first place of malignant tumor.18F-FDG PET/CT is widely used in the diagnosis of pulmonary malignant tumor accurate non-invasive imaging method.PET is usually due to low spatial resolution limits caused by high false negative results, the evaluation of sub centimeter for NSCLC is still not clear, especially for lepidic growth oriented small adenocarcinoma. On the contrary,we have previously reported that 18F-FDG PET/CT predict the potential role of sub centimeter NSCLC lymph node metastasis, and CT lesions in thin solid components are closely related. So even for sub centimeter lesions,18F-FDG PET/CT may be a prediction method for quantitative evaluation of potential malignant tumor promising. However, there are few studies on 18F-FDG PET/CT to assess the clinical significance and prognosis of nodules less than 1cm in diameter. In order to further evaluate sub centimeter tumor patients with NSCLC malignant potential and predicting the prognosis,it is necessary to elucidate the clinical pathological characteristics, prognosis and appropriate operative treatment. Therefore this study retrospectively analyzed the clinical PET examination before operation. The significance of surgical resection of clinical stage PET/CT check NO sub cm NSCLC'8F-FDG patients.PurposeObjective to investigate the clinical significance and prognostic value of 18F-FDG PET/CT in patients with non small cell lung cancerResearch purposes and MethodsRetrospective analysis of 2010-2015 in our hospital PET/CT 189 cases of clinical stage NO sub cm NSCLC patients, preoperative PET examination and measurement of SUVmax.pathological invasion(PI)is considered to be one of the following factors:at least with lymphatic invasion, vascular invasion, pleural invasion and lymph node metastasis. The survival curves and log rank Kaplan-Meier check the survival rate was calculated.ResultThe mean SUVmax was 1.7 ± 1.8(SUVmax ranged from 0.6 to 13.0)with a median of 1.0.28(15%)sub-centric NSCLC patients with PI.Multivariate regression analysis showed that SUVmax was an independent and important clinical indicator for predicting PI(P =0.0251)and overall survival(OS)(P = 0.0485).The ROC curve analysis showed that SUVmax = 2.0 was used as the threshold for predicting PI.Compared with the low SUVmax group, the high SUVmax group(SUVmax>2.0;n = 42)showed more solid lesions(91%vs 14%;p<0.0001)and postoperative lymph node invasion(12%vs 0%;p&It;0.0001).There were significant differences in lung cancer-specific OS(LCS-OS)between the high SUVmax group and the low SUVmax group(LCS-OS were:(92.3%vs 96.9%;p = 0.0054),and recurrence of cancer on TLC In the study, the SUVmax group had a better 3-year recurrence-free survival(RFS)(88.3%vs 50.0%;p = 0.0453).Conclusion1.PET/CT examination of SUVmax can reflect the invasiveness of tumor2.PET/CT has a significant effect on the prognosis of sub NSCLC,especially in the thin layer of CT on the pure solid tumors.Part ?Prognostic value of 18F-FDG PET/CT whole body imaging in the treatment of non-small cell lung cancerBackgroundLung cancer is the incidence and mortality of malignant tumors with the highest.2015 years,in China there are about 4 million 292 thousand new cases of cancer, including lung cancer, with 733 thousand new cases(total 17.1%)to become the people with cancer China or because of the biggest threat to cancer death. In primary lung cancer is about 80%for non small cell lung cancer, clinical studies show that tumor induction therapy, the therapeutic effect of 18F-FDG and tumor therapy PET/CT measured lung cancer primary active site after glucose metabolism. On PET/CT images in the study during induction chemotherapy by primary tumor or metastatic lymph node changes in glucose metabolism and the role of the prediction of tumor efficacy rarely.PurposePrediction of therapeutic efficacy and survival of patients with non small cell lung cancer by 18F-FDG PET/CT whole body imagingResearch purposes and MethodsA retrospective analysis of 2010-2015 in non-small cell lung cancer surgery in our hospital before treatment or after treatment and induction of PET/CT examination of patients. The gross and microscopic total resection surgery were divided into R0,under the microscope of incomplete resection R1,incomplete resection is the survival of the naked eye R2.(OS)from the operation time at the time of death or end of follow-up time.Collect diagnostic biopsy or biopsy confirmed lymph node metastasis in N2 patients. The primary tumor and lymph node metastasis N2 stage changes in SUVmax and SUVmax percentage after induction therapy.Calculation of lung cancer primary part of SUVmax is obtained from.SUVmax into Xeleris workstation induction therapy before or after induction therapy.Calculation of induced changes before and after treatment PET SUVmax absolute value and percentage point cut.Preoperative and postoperative SUVmax, change the four percentile cut-off point for the percentage of SUVmax,0 SUVm Ax absolute value change(SUVmax increase vs no change or decrease)of the cut-off point,to evaluate the predictive factors of survival.ResultDuring the treatment of lung cancer induced by primary standard uptake value(SUV)linked to increased overall survival after surgery decreased. Both before the treatment of the primary tumor site or change the percentage of the absolute value of SUV and the postoperative overall survival in patients with non-small cell lung cancer is indpendent of the SUV to reduce the overall survival is greater than 60%the best predictor of stage.N2 mediastinal lymph node metastasis, predicting overall survival changes than the primary tumor site SUV better.Conclusion1?The prognosis of patients with non-small cell lung cancer(SUV)after induction therapy increased significantly compared with patients with stable or reduced SUV2?The change in glucose uptake of mediastinal lymph node metastasis was a better predictor of survival compared with changes in glucose uptake in the primary site.
