| ObjectiveTo investigated the effects of different frequency(2Hz,100Hz)elec-troacupuncture at back-shu points on patients with chronic fatigue syndrome.At the same time,to discuss its effects on Behavior and HPA axis and immune in chronic fatigue Rats.MethodsThis research was divided into two parts:clinical study and animal experimental study:1.Clinical study:92 patients with chronic fatigue were randomly divided into three groups:31 cases in 100Hz group,30 cases in 2Hz group and 31 cases in routine acupuncture group.100Hz(100Hz,1mA)group' s acupoints:Feishu,Ganshu,Xinshu,Pishu,Shenshu.The selection of points in 2Hz(2Hz,1mA)group was the same as the 100Hz group.The selection of points in routine group was the same as the 100Hz group and 2Hz group.After 2 courses of treatment,according to each group of patients with fatigue scores,depression scores,anxiety scores,SF-36 scores and 33 clinical symptoms' scores to evaluated.2.Animal experiment:Using the method of sleep deprivation with exhaustive swimming and chronic restraint to cause the chronic fatigue model in rats.The rats were randomly divided into 100Hz group(n=12),2Hz group(n=12),conventio nal acupuncture group(n=12),model group(n=12),normol group(n=12)with the SPSS20.0.100Hz group' s acupoints:Feishu,Xinshu,Pishu,Ganshu,Shenshu.The selection of 2Hz group was the same as the 100Hz group.The selection of conventional acupuncture group was the same as the 100Hz group and 2Hz group.The above groups were treated while continuing with the previous modeling stimulus.Normal group was without any intervention,and the model group was without any treatment.The behavioral should be assessed before and after modeling and treatment.The levels of inflammatory factors and levels of ACTH?CORT in the blood were measured by enzyme-linked immunosorbent assay(ELISA).The expressions of 5-HT1aRmRNA and CRHmRNA in Hypothalamus were measured by qPCR.Result:1.Fatigue score:After one course of treatment,the 2Hz group' s score was lower than before,which had significant statistical significance(P<0.05),the 100Hz and conventional groups' scores were the same as before,which had no significant statistical significance(P>0.05);to compared among groups,2Hz group was lower than 100Hz group and conventional acupuncture group(P<0.05),100Hz group' s score was the same as the conventional acupuncture group(P>0.05).After two courses of treatment,the fatigue scores of the three groups were significantly lower than those before(P<0.05).The 2Hz group was lower than 100Hz group,and the 100Hz one was lower than the conventional acupuncture group with statistically significance(P<0.05).2.Depression score:After one course of treatment,the 2Hz and 100Hz groups' scores were lower than before,which had significant statistical significance(P<0.05),the score of conventional acupuncture group was not significantly lower than before(P>0.05);to compared among groups,2Hz group was lower than conventional acupuncture group,the difference was statistically significant(P<0.05),the difference between 2Hz group and 100Hz was not significant(P>0.05),the difference between 100Hz group and conventional acupuncture was not significant(P>0.05).After two courses of treatment,the depression scores of the three groups were significantly lower than before(P<0.05).The 2Hz one was lower than 100Hz group,and the 100Hz one was lower than the conventional acupuncture grcoup with statistically significance(P<0.05).3.Anxiety score:After one course of treatment,the 2Hz group' s score was lower than before,which had significant statistical significance(P<0· 05).The 100Hz and conventional groups' scores were the same as before,which had no significant statistical significance(P>0.05).To compared among groups,2Hz group was lower than conventional acupuncture group(P<0.05),the difference between 2Hz group and 100Hz group was not significant(P>0.05),the difference between 100Hz group and conventional acupuncture group was not significant(P>0.05).After two courses of treatment,the scores of the three groups were significantly lower than before(P<0.05).The 2Hz one was lower than 100Hz group,and the 100Hz was lower than the conventional acupuncture group with statistically significance(P<0.05).4.SF-36 score:After treatment,eight dimensions scores were significantly higher than before,the differences were statistically significant(P<0.05).?In the aspects of GH?PF?RP?RE and SF:to compared among groups(integrals):2Hz group was higher than 100Hz group,100Hz group was higher than conventional acupuncture group(P<0.05);to compared among groups(differeces):in the aspects of GH?RP,the 2Hz one was higher than 100Hz one,100Hz one was higher than the conventional acupuncture group(P<0.05);in the aspects of PF?RE,2Hz group was the same as 100Hz group(P>0.05),and they were higher than the conventional acupuncture group(P<0.05);in the aspect of SF,2Hz group was higher than 100Hz group and conventional acupuncture group(P<0.05),100Hz group was the same as conventional acupuncture group(P>0.05).?In the aspects of VT and MH(differeces and integrals),there were no significant differences between 100Hz group and 2Hz group(P>0.05),they were all higher than the conventional acupuncture group with statistically significance(P<0.05).?In the aspect of BP(differece and integral),the difference among them had no statistically significance(P>0.05).5.Thirty-three clinical symptoms' scores:?the 100Hz electroacupuncture with a relative therapeutic advantage:forgetfulness?dry mouth?joint pain?loose stools,the scores were significantly lower than before(P<0.05),100Hz group was lower than 2Hz group and conventional acupuncture group(P<0.05).To compared among groups(differences),in the aspect of dry mouth,100Hz group was lower than conventional acupuncture group(P<0.05),the difference between 100Hz and 2Hz group was not significant(P>0.05),the difference between 2Hz group and conventional acupuncture group was not significant(P>0.05).In the aspect of joint pain,100Hz was lower than 2Hz group(P<0.05),the difference between 100Hz and conventional acupuncture group was not significant(P>0.05),the difference between 2Hz and conventional acupuncture group was not significant(P>0.05).In the aspect of loose stools,100Hz group was lower than conventional acupuncture group and 2Hz one(P<0.05),2Hz group was the same to conventional acupuncture group(P>0.05).In the aspect of forgetfulness,the difference among groups was not significant(P>0.05).?The 2Hz electroacupu ncture with a relative therapeutic advantage:fatigue,muscle pain or soreness discomfort,depression,palpitation,allergies,neck axillary lymph nodes,night sweats,the feeling of muscle strength,headache,fullness discomfort of chest,constipation,loss of appetite,unresponsive,inattention The scores were lower than before(P<0.05),and were lower than 100Hz group and conventional group(P<0.05);to compared among groups(differences),in the aspect of the feeling of muscle strength,2Hz one was lower than 100Hz group(P<0· 05),100Hz group was lower than conventional group(P<0.05).In the aspects of fatigue,headache,fullness discomfort of chest,constipation,2Hz one was lower than 100Hz group and conventional group(P<0.05),the difference between 100Hz group and conventional group was not significant(P>0.05).In the aspects of muscle pain or soreness discomfort,palpitation loss of appetite,unresponsive,2Hz was lower than conventional group(P<0.05),the difference between 2Hz one and 100Hz group was not significent(P>0.05),the difference between 100Hz group and conventional group was not significant(P>0.05).In the aspects of depression,allergies,neck axillary lymph nodes,night sweats,inattention,the differences were not significant(P>0.05).?The scores and differences of throat' s pain,abdominal distension and anesthesia,sleepiness in 2Hz and 100Hz groups were lower than those in conventional group with statistically significant(P<0.05),but there was no significant difference between them.? The scores and differences of irritability,anxiety,sorrow,insomnia,nausea,tonic acid in three groups were significantly lower than before(P<0.05),and there was no significant difference among groups(P>0.05).?Three groups of patients with dizziness,belching or hiccups,spontaneous perspiration,hypothermia,cough,weeping shame' s scores were the same as before,which with no significant reduction(P.>0.05).The total scores of the three groups of clinical symptoms were significantly reduced(P<0.05),the 2Hz group was lower than the 100Hz group,the 100Hz group was lower than the conventional acupuncture group with statistically significance(P<0.05).And the change of differences in three groups were the same as scores.6.Behavioral results:Compared with the normal group,the speed of weight gain of rats in the model group,the 100Hz group,the 2Hz group and the conventional acupuncture group were slower,the time of exhaustive swimming were shortened,the tail suspension time was prolonged,central lattice for a long time,the times' of horizontal and vertical movements were decreased,the difference was statistically significant(P<0.05).After treatment,100Hz group,2Hz group and conventional acupuncture group compared with the model group,the speed of gaining of weight was faster,the time of exhaustive swimming was increased,the time of tail suspension was shorted,the times' of horizontal and vertical movements were increased,the difference was statistically significant(P<0.05).7.The results of inflammatory factor levels:After treatment,compared with the model group,the level of IL-4 was decreased and the level of IFN-? was increased in three treatment groups,the differences were statistically significant(P<0.05);the ratio of IFN-?/IL-4 was increased(P<0.05),and the 2Hz group was higher than the 100Hz and conventional acupuncture group(P<0.05).The level of IL-2 in the 2Hz group was higher than that in the 100Hz group and conventional acupuncture group(P<0.05),and there was no significant difference in IL-10 level in 100Hz group,2Hz group and conventional acupuncture group(P>0.05),but they were all lower than the model group.8.The levels of ACTH and CORT in the serum:The levels of ACTH and CORT in the serum of the three treatment groups were significantly lower than those in the model group(P<0.05).In the aspect of ACTH,the 2Hz group was lower than the conventional acupuncture group(P<0.05),there was no significant difference between 100Hz group and conventional acupuncture(P>0.05),there was no significant difference between 100Hz group and 2Hz group(P>0.05).In the aspect of CORT,2Hz group was lower than that in 100Hz group,and 100Hz group was lower than that in the conventional acupuncture group(P<0.05).9.The expressions of 5-HT1aRmRNA and CRHmRNA:The expression of 5-HT1aR mRNA in all the treatment groups were significantly higher than those in the model group(P<0.05),to compared among groups,2Hz group was higher than 100Hz group(P<0.05),100Hz group was the same as the conventional acupuncture group(P>0.05).The expression of CRHmRNA in treatment groups were lower than the model group(P<0.05),to compared among groups,2Hz group was lower than 100Hz group,100Hz group was lower than the conventional acupuncture group(P<0.05).Conclution:The treatment of different frequency electroacupuncture at back-shu points had obvious advantage to chronic fatigue syndrome.And the 2Hz one was better.Animal experiment showed that electroacupuncture can not only increased the body weight and behavioral activity of rats,but also regulated the immune function to prevented the conversion to Th2,and regu lated the levels of ACTH?CORT in serum of the rats and the expressions of 5-HT1aRmRNA and CRHmRNA in the hypothalamus;clinical study showed that electroacupuncture can decrease the states of the patients' fatigue,depression,anxiety,improve the quality of life and activity of patients,the 2Hz electrical was better,which was the same as the animal experiment. |
