How to design a way to improve the sensitivity and specificity of targeted diagnosis and therapy for intractable disease,is one pressing problem for areas of diseases diagnosis and therapy.In this paper,on the basis of the traditional scintigraphy,we use the our own studied optimization techniques of targeted diagnosis and therapy for functional metabolic scintigraphy(FMS)to build a series of methods which can improve effectiveness of diagnosis and treatment,and successfully applied these methods to clinical :1)Noninvasive gated cardiac FMS in the targeted diagnosis of latent coronary heart disease(LCHD).LCHD is very easy to be misdiagnosed.Therefore,by using the noninvasive gated FMS targeted diagnosis technology,we screened out cardiac diastolic function parameters,which has confirmed value for LCHD.At the same time,covariance analysis can eliminate age interference.The results show that this FMS,combined application of loaded and quiescent myocardial tomography FMS,it can improve the specificity and accuracy of diagnosis.By using the noninvasive gated cardiac function FMS,analysis of pump function parameters for whole and local left ventricle,we think it can improve the diagnostic sensitivity.Most importantly,it reveals the specific physiological function changes in the LCHD.2)Cerebral blood perfusion FMS in targeted tomographic diagnosis of the epileptic focus.We applied the FMS to detect local blood perfusion changes in cerebral cortex tissue.By using ? knife treatment clinical results of a long-term follow-up epilepsy patients as gold standard,"seizure has stopped" as threshold of diagnostic tests,we carried out diagnostic tests and evaluated the cerebral blood perfusion FMS targeting epileptic focus.The results showed that this method has advantage of high sensitivity and accuracy for targeted diagnosis,and can be used as surgery for drug refractory epilepsy or stereotactic radiosurgery treatment.It also can provide imageology basise for epileptic focus.3)Non-invasive hepatobiliary FMS in targeted diagnosis of the reflux gastritis.Routine used traumatic examination method to diagnose reflux gastritis,is difficult to avoid false positives/negatives.Therefore,we applied non-invasive hepatobiliary FMS to diagnosis of the disease.Besides,the image data collection employ optimizational technologies such as "physiological intervention and labeled target organs".According to pathological diagnosis results,we evalue FMS diagnostic testsand ROC analysis.The results show that this method can accurately diagnose reflux gastritis,having advantage of significantly improving target diagnostic efficiency.In addition,the study confirmed that the patients' excretion function of gallbladder does not reduce;But the pyloric sphincter abnormal function may be one of the main mechanisms leading to the occurrence of reflux gastritis.4)FMS and other multimodal imaging guided 125 I particle targeted therapy of nonsmall-cell lung cancer(NSCLC).How can effective treatment NSCLC patients with the operative,radiotherapeutic,chemotherapeutical contraindications has been a clinical problem.Therefore,we adopt multimodal imaging such as FMS,CT,18F-FDG coincidence FMS,bronchoscopy(FFB)to locate the target area;according to the treatment planning system(TPS)to make optimization of radiation dosimetry and dose volume histogram(DVH)to evalue TPS;using imageologic to guide particals implantation,reaching the purpose of real-time location verification,dosimetric verification after implantation,follow-up therapeutic efficacy.The results show that imageologic target area of tissue density and biological target area is clearly defined to ensure the targeted therapy and conformal property;QA/QC includs radiation dosimetry optimization can significantly improve therapeutic gain ratio and local control effect,thereby significantly reduced toxic side effectoccurs.
