Objective: To study the therapeutic effect of early arthroscopic surgery intervention primary frozen shoulder.Methods: Using randomized control study, from June 2014 to August 2015, early primary frozen shoulder patients admitted for conservative treatment or arthroscopic surgery are a total of 68 cases, with an average follow-up of 12months(12 to 14 months).There were 45 females; 23 males, mean age 53.5 years(40 to 70 years). In accordance with primary frozen shoulder Nevaiser staging, stage?patients 36 cases, female 25 cases, male 11 cases;. stage?patients 32 cases, female 20 cases, male 12 cases. Stage? and ? patients were randomly divided into conservative treatment(group A) and arthroscopic treatment(group B) each other. There were group A 20 cases and group B 16 cases in stage?; There were group A 14 cases and group B 18 cases in stage?. Evaluation using visual pain scale(VAS), the American Shoulder and Elbow Surgery-Standard shoulder score(ASES), shoulder active range of motion(ROM). There was no significant difference in age and sex between group A and group B in stage???.Results:(1)After 3 weeks of treatment of primary frozen shoulder stage???, there are significant differences between grop A and group B in VAS score, prompted conservative treatment(group A) relieving shoulder pain better than arthroscopy(group B). After 3 months?1 year of treatment of primary frozen shoulder stage???, there are significant differences between grop A and group B in VAS score, prompted arthroscopy(group B) relieving shoulder pain better than conservative treatment(group A).(2) After 3 weeks of treatment of primary frozen shoulder stage?, there are significant differences between grop A and group B in shoulder active range of motion(ROM), prompted conservative treatment(group A) improving shoulder active range of motion better than arthroscopy(group B). After 1 year of treatment of primary frozen shoulder stage?and 3 months?1 year of treatment of primary frozen shoulder stage?, there are significant differences between grop A and group B in shoulder active range of motion(ROM), prompted arthroscopy(group B) improving shoulder active range of motion better than conservative treatment(group A).(3) After 3 weeks of treatment of primary frozen shoulder stage???, there are significant differences between grop A and group B in ASES score, prompted conservative treatment(group A) ASES score better than arthroscopy(group B). After 3 months of treatment of primary frozen shoulder stage?and 1 year of treatment of primary frozen shoulder stage???, there are significant differences between grop A and group B in ASES score, suggesting that arthroscopic treatment(group B) to improve the daily life function better than conservative treatment(group A). There were no serious complications occurred in patients with primary frozen shoulder stage? conservative treatment(group A) 1 case lost.Conclusions: For early primary frozen shoulder, especially Nevaiser stage ?, arthroscopic capsular release, inflammatory synovial cleaning, after NSAID drugs and physical therapy can significantly improve the shoulder range of motion, relieve pain, fewer complications, and can effectively improve the daily lives function. The disease is not easily repeated.
