| Professor Li Siwen is the expert instructor of the fourth and fifth national traditional Chinese academic experiences inheriting work. He is also a notable Chinese medicine doctor in Yunnan Province and a PhD tutor of Chengdu University of Traditional Chinese Medicine. Having been engaged in clinical medicine, teaching and research work for nearly 40 years, he has formed his unique ways of diagnosis and treatment experiences at cancer and other difficult diseases by using TCM and Integrative Medicine.This Thesis is based on Professor Li's academic origin, combined with my three years clinical practice by following the master, at same time summarizes the major academic ideas and clinical experiences of treatment of malignant tumors. It is mainly focused on Professor Li's treatment principal of non-small cell lung cancer. The paper is divided into three parts:Part One:Professor Li's Academic Thoughts in Identifying Cancer Patterns and their Origin1. Organizes Professor Li's Academic Thoughts in Identifying Cancer Patterns and their Origin.2. Summaries Professor Li's Academic Thoughts in Identifying Cancer Patterns.(1) Professor Li thinks that cancer occurs because "Deficiency of human body's upright Qi is essential, tumor is a condition and the imbalance is the key". These three are indispensable.(2) Professor Li advocates "Tonifying the upright Qi and eliminating the tumors by using both supplementation and sedating; employing various TCM methods to cure and assisted by western medicine in different stages" It presents the individual cancer treatment principles and strategies in the way of TCM.(3) For the advanced or elderly cancer patients or with weak constitution, Professor Li's treatment idea is "Human -oriented and to live harmonious with tumor"(4) For the fact that prevention is more important than treatment at oncology, Professor Li is carrying forward the spirit of Chinese medicine on "First rate doctors treat disease before its happening" and focusing on prevention. By emphasizing early medical intervention in the groups at high risk of cancer and patients after tumor surgery and chemotherapy, the therapeutic focus is moved to the earlier stage in order to reduce tumor development, metastasis and risk of recurrence.(5) Professor Li is also good at promoting the use of therapeutic and psychological treatment for patients'recover. "Mental healing and medicine simultaneously" plus "Food together with medicine" bring benefits to countless patients and fully demonstrate what a noble medical doctor my teacher is.Part two:Summary of clinical experiences of Professor Li's treatment of cancer and complicated diseases.1. From the pathogenesis, treatment according to the patterns, herbal prescription experience and other aspects form into a comprehensive summary of Professor Li's clinical experiences of common cancer diagnosis and treatment.(1) Colon cancer starts from deficiency of upright Qi, spleen and kidney deficiency, plus heat and dampness in the lower burner forming stasis. So the treatment should be based on clearing heat, eliminating dampness, detoxifying and softening the hardness, moving Qi and resolving stagnation, then tonifying upright Qi. (2) Breast cancer is due to liver and spleen not in harmony, liver and kidney deficiency, causing Qi and blood stasis. So the treatment should be focused on "Clearing the up and supporting the down", which means "clearing up"" is to detoxify and clearing the heat; "supporting down" is to regulate live and spleen or nourishing liver and kidney. (3) Liver cancer is caused by live and spleen not in harmony, the key lies soothing the liver and tonifying spleen, adjusting qi. (4) Laryngeal cancer is due to deficiency heat, so the treatment should clear up heat and do supplementation. It is necessary to clear toxic heat in the lung at same time nourish lung Yin, preventing deficient fire from within. (5) The occurrence of brain tumor is more due to phlegm, can be combined with stasis, toxic and fire. Treatment should transfer phlegm and unblock the meridians, pacify liver and eliminate wind, at same time nourish the liver and kidney. (6) Urethral cancer is a disease with deficient root and access symptom. Its root is yin deficiency and spleen kidney deficiency; symptom lies heat and dampness forming stasis in lower burner; by using herb formulas having various methods to tonify spleen and kidney, clear heat and transfer dampness, purg fire and remove stasis.2. Professor Li's clinic experience by treating miscellaneous diseases.(1) Unexplained summer fever, more from a mixture of long summer heat and humidity. It blocks the qi flow, so the treatment should be clearing heat and resolving dampness, using fragrant herb to disperse the turbid and detoxification. (2) Postpartum lochia should be treated with warming meridians and dispersing cold, nourishing blood and removing stasis. (3) Children with intractable herpes should be treated with tonifying qi, moving blood and dis-pelling wind. (4) For huge ulcers on the waist caused by radiotherapy, using tonifying and sedating, both clearing and supplying.Part Three:Study of Professor Li's diagnosis and treatment of non-small cell lung cancer.By retrospective clinical research method, using frequency analysis, cluster analysis, correlation analysis and mathematical statistics to study and summarize Professor Li's treatment of non-small cell lung cancer from the view of Symptom-Pattern-Formula-herb. This research provides scientific evidence for the diagnosis and treatment of non-small cell lung cancer in the way of traditional Chinese medicine.1. Professor Li thinks loss of Yin liquid is the fundamental pathological basis of non-small cell lung cancer, and this can go through the whole process of disease. When the disease lingers over a long period, the loss of Yin can further injure Qi and causes Qi deficiency or Qi Yin deficiency. Another important factor for the occurrence and development of non-small cell lung cancer is the cancer toxic.Its nature is mostly "toxic heat", can be combined with phlegm and stasis. This disease is located mainly in the lungs, but spleen and kidney are also closely related.2. Professor Li thinks non-small cell lung cancer mainly mixed with access and deficiency, often the access and deficiency comes alternatively. Deficiency usually consists Yin deficiency (including Lung Yin and Kidney Yin deficient) and Qi deficiency (including lung qi and spleen qi deficient). Access mainly contributes to toxic heat, blood stasis, phlegm dampness, Qi stagnation and water retention.3. Professor Li's treatment of non-small cell lung cancer is divided into four patterns:toxic stasis blocked within, Qi deficiency and phlegm dampness, yin deficiency with heat toxic, both Qi and Yin deficiency. Specific formulas:for toxic stasis using yinqiao taohong tang with change; for Qi and phlegm dampness using Liu Junzi tang with change; for Yin deficiency and heat toxic using yinqiao shamai tang with change; for both Qi and Yin deficiency using shasheng maidong tang with change.4. Professor Li focuses on the main symptoms to give herbalPrescription, thereby to increase the effect and accuracy of clinical diagnosis and treatment. Patients with cough, red tongue and rapid pulse are given yinqiao Shamai tang to nourish Yin clear heat and detoxify; If patients have fatigue, poor appetite and pale tongue, then Liu Junzi tang is used to tonify Qi and spleen; If the main symptoms are cough, body pain and tongue is dark with stasis, Yinqiao Taohong tang is often used to detoxify and resolve stasis, unblock the meridians and stop the pain; If the main symptoms is cough, not much phlegm and fatigue, Shasheng Maidong tang should be used to tonify Qi and nourish Yin.5. The most frequently used herb, paired herbs and combination of herbs by Professor Li's for the treatment of non-small cell lung cancer are tonifying herbs (mostly tonify Qi and nourish Yin). The less frequently used herbs are for clearing heat (mostly clear heat and detoxify). For the herbs which invigorating blood, resolving stasis, unblocking meridians, moving Qi and transfer phlegm stopping asthma are also necessary. From the herbs he use we come to know that tonifying, clearing heat, transferring phlegm and unblocking meridians are the main treatment methods which professor Li employs. These herbs and herbal combinations reflect Professor Li's treatment principal of "Tonifying the upright Qi and eliminating the tumors, supplementing and sedating; employing various TCM methods to cure. " from different angles.6. Through data digging combining experience by following my teacher, I have summed up Professor Li's acknowledge of pathogenesis for non-small cell lung cancer-"Yin liquid deficiency is the root, phlegm heat and stasis toxic is the symptom. In therapy, my teacher follows the principal of " Treating with various TCM methods, focusing on clearing and tonifying, treat with different stages. The use of herbal Prescription has a clear order and with thoughtful combination. Through analyzing the characteristics of how my teachers uses herbs, we can see the way he gives prescription is quick and cautious, unique and sophisticated. His selection of herbs is both according to the TCM tradition, and combining with development of modern medicine. Especially his treatment concept of "individual" and " co-live with the tumors " reflects unique advantages of TCM way of treatment for non-small cell lung cancer.As can be seen from the above summary of research:Professor Li's diagnosis and treatment concepts for oncology is rich and wide, has unique clinical experience. He considers deficiency of upright Qi and cancer toxic playing an important role in non-small cell lung cancer pathogenesis. He holds the treatment concept of Tonifying the upright Qi and eliminating the tumors, supplementing and sedating; employing various TCM methods to cure, focusing on light tonifying.He formed his own characteristics and experiences in treating non-small cell lung cancer with TCM diagnosis, pattern, prescription and herb, also so far he has achieved fairly good therapeutic effect. It is worth our further study, researching, systems, analysis, and further innovation on the basis of preserving. |
