The Study Of Jiangzhi Tongmai Pellet To Cure Coronary Stenosis Patients And To Influence The Plaque In Coturnix AS Model

Objective:To observe the clinical effects of Jiangzhi Tongmai tablet on the coronary stenosis patients and to explore the influence of Jiangzhi Tongmai tablet on coturnix AS models and its related mechanisms.Method:First,31 selected patients were randomly divided into treated group (n=16) which was treated with Jiangzhi Tongmai tablet and control group (n=15) which was treated with Simvastain tablet. The curative effects of angina pectoris and syndromes of TCM(traditional Chinese medicine) and the changes of electrocardiogram, heart function (SV、SI、CI、EF、E、A), blood-lipid(TC、TG、LDL-C、HDL-C), lipoprotein (ApoA1, ApoB), hs-CRP, IL-6, TNF-a, MMP-9 and the results of CAG were observed. Second, cotunix AS models were built and then classified according to different qualities of cotunix and were randomly divided into seven groups(n=19 each group) i.e. control group and unoperated group treated with distilled water, three groups treated with liquid medicine of powdered Jiangzhi Tongmai tablet in high (1.56g/kg) medium(0.78g/kg), low dosage (0.39g/kg)(which equals to the dosages of raw medicinals of 8、4、2g/kg), Simvastain group(4mg/kg), Zhijiangning group(1.2g/kg).The medicinals were given through gastry lavage. Then, the immuno-histochemistry of blood-lipid(TC、TG、LDL-C. HDL-C), lipoprotein (ApoA1、ApoB), hs-CRP, IL-6, TNF-a, VCAM-1, MMP-9. the homogenate lipoid examination of the cardiac muscle and the liver tissues, and the pathological examination of the arteriae and heart tissues.Result:The effective rates calculated according to the curative effects of angina pectoris, syndromes of TCM(traditional Chinese medicine) and the changes of electrocardiogram are 87.50%,93.75%,81.25%,which are obviously higher than the control group which are 60.00%,73.33%, 66.67%(P<0.05). The changes of electrocardiogram, heart function (SV、SI、CI、EF、E、A),blood-lipid(TC、TG、LDL-C、HDL-C), lipoprotein (ApoA1, ApoB), hs-CRP, IL-6, TNF-a, MMP-9 and the results of CAG of the two groups were significantly improved(P<0.05), and the treated group is superior to the control group(P<0.05 or P<0.01).The improvement of the immuno-histochemistry of blood-lipid(TC、TG、LDL-C、HDL-C), lipoprotein(ApoA1, ApoB), hs-CRP, IL-6, TNF-a, VCAM-1, MMP-9 and the pathological examination of the arteriae and heart tissues by treating coturnix AS models with Jiangzhi Tongmai tablet are obvious. And statistically the diferences between the Jiangzhi Tongmai group and control group are significant(P<0.05 or P<0.01), especially the high dose group.Conclusion:1 Jiangzhi Tongmai tablet not only can relieve the clinical symptoms and the ischemic electrocardiogram, but also regulate the blood-lipid, inhibit inflammatary factors and reduce coronary artery scoring.2. It reduces the TC and TG level, raise the ApoA1 and regulate the lipid metabolism in AS models.3. It lowers the content of hs-CRP, IL-6 in serum, inhibt the expression of the immune factor VCAM-1 on the artery wall and the inflammatory reaction.4.It downregulates the degradation factor of the plaque fibrous cap(expression of inflammatory factor and MMPs),and prevent the plaque fibrous cap continuously turning thin to stabilize the plaque.5.It relieves the severity of atherosclerosis and reduce the thickness of endarterium to eliminate the paque in coturnix AS models.
