Objective To analyze the distribution of Chemerin between synovial fluid(SF) and serum of patients with knee osteoarthritis(OA), discuss the association of Chemerin levels with the disease severity of osteoarthritis and synovitis, provide theoretical basis for the early diagnosis of osteoarthritis.Methods 100 knee surgery patients(100 knees) were collected in our hospital from January to December 2013, in which the clinical diagnosis of OA patients(according to the 2001 American College of Rheumatology diagnostic criteria on OA) 80 cases of 80 knees(OA group), the other 20 knees of 20 cases of knee joint disease(non-OA group). Levels of Chemerin were detected by an enzyme-linked immunosorbent assay in SF and serum of these patients. Knee OA grading was evaluated according to the Kellgren-Lawrence classification. And the degree of synovitis was evaluated on basis of Ayral synovitis score Method.Results Chemerin level of OA group in serum and synovial fluid(103.19±6.52 ug/l;128.35±5.31 ug/l) was significantly higher than that of control group(98.61±0.52 ug/l;92.52±4.56 ug/l), with statistical difference(P<0.01). The level of Chemerin in serum and SF in patients of OA had a positive correlation with serum CRP(r=0.62;r=0.78,P<0.05). Serum Chemerin level was positively correlated with Ayral score(r=0.538,P=0.032),and synovial fluid Chemerin level was positively correlated with Ayral score(r=0.692,P=0.024);Serum Chemerin level of OA patients was positively correlated with Ayral score(r=0.331,P<0.01),and synovial fluid Chemerin level of OA patients was positively correlated with Ayral score(r=0.375,P<0.01). The level of Chemerin in SF and serum in patients of OA were positively correlated with the Kellgren-Lawrence classification(r=0.981,P<0.001;r=0.901,P<0.001). And there was a positive correlation between the level of Chemerin in SF and in serum(r=0.596,P<0.01). The level of Chemerin in SF and serum in patients of OA were positively correlated with body mass index(BMI).Conclusions Levels of Chemerin in SF and serum of 100 patients reflect the degree of synovitis. Levels of Chemerin in SF and serum were significantly increased in patients with OA, and levels of Chemerin were positively correlated with severity of OA.Chemerin levels can be used as a reference biomarker of diagnosis and severity of OA.
