Aim:To investigate the link between the clinical heart rate and outcomes durging and after PCI in cardiovascurlar disease patients.Materials and methods:This study is an single center, retrospective, observational, cohort Study in CAD patients which aged from 18 to 80 year-old, diagnosed as coronary heart disease with records on mean clinical heart rate during PCI from 2009.1.1 to 2014.12.31 in the database from the department of cardiology, Chinese PLA General Hospital, Beijing. All 10519 subjects’related data from the database was collected from medical records. Patients with CAD proven by coronary arteriography and were given PCI were enrolled. Exclusion criteria for the registry consisted of the following:1. Inferior wall myocardial infarction (MI) history or/and acute inferior wall MI patients; 2、echocardiography showed LVEF< 30%; 3、patients Installed a permanent pacemaker. Information collected at baseline included: demographics; risk factors and lifestyle; results of physical examination; HR (determined by pulse palpation at the admitted day); laboratory values (e.g. serum creatinine, hemoglobin, and cardial troponin T); and β-blocker treatments during hospitalization. Heart rate in cardiac catheterization room was recorded by nurses. The anonymised demographic, clinical and prescribing data was collected by review medical records. This study was registered in the clinical trials with the number NCT02351674. The primary efficacy endpoint is proportion of patients with composite events including CVD mortality, fatal/non-fatal MI, non-fatal sudden cardiac arrest, fatal/non-fatal stroke and malignant arrhythmia.Results:In 8179 patients, The mean resting HR was 73.87 ± 14.36bpm when the patient was admitted in hospital. The mean clinical heart rate during PCI was 70.56± 11.48 bpm, According to the mean clinical heart rate during PCI, the patients were divided into four groups.The cardiovascular events of four groups were 110(2.5%), 77 (3.2%),,45 (4.8%),42 (9.7%) respectively.(P< 0.0001). Multivariate regression analysis revealed that females、multiple vessels disease、anemia、elevatation of TnT. high mean clinical heart rate during PCI were independent risk factors for cardiovascular events in selective PCI patients. In directive PCI patients, independent risk factors for cardiovascular events in DSA room were high mean clinical heart rate during PCI.Conclusion:High mean clinical heart rate during PCI were independent risk factors for cardiovascular events in selective PCI patients.
