Taiwan Area Law Of Diabetes And Syndrome Of TCM

Objective:To investigate the etiology and pathogenesis of diabetes, occurrence regularity and the characteristics of TCM syndrome, improve various syndromes of diabetes mellitus of internal medicine clinical differentiation level, promote the prevention and treatment of diabetes of TCM clinical effectiveness, and provide research basis of treatment, therapies and drug prescription.Methods:We use theoretical research integrated with clinical case studies to conduct a thorough investigation of pain types, supplemented by our own clinical investigations as follows:1. Theoretical study:The research based on "Huandi’s canon of Medicine", "Treatise on Exogenous Febrile Disease", and analysis of the other ancient texts, as well as modern TCM works of the last ten years to study on the origin of diabetes, etiology and pathogenesis, syndrome features, the therapeutic principle, and discuss the important status and significance in clinical medicine in diabetes mellitus.2. Clinical investigation:a number of diabetes outpatient case studies from Taiwan TCM practice are presented, analyzed and discussed.Results:1. Theoretical study:From analysis of the "Huandi’s canon of Medicine", "Treatise on Exogenous Febrile Disease" and other ancient texts, as well as modern TCM work of the last ten years, we found that the causes of diabetes mellitus are Improper diet, mood disorder, endowment deficiencies, overwork, and other exogenous factors leading to diabetes. Its pathogenesis is because of 1. "weakness pathogenic"(Qi deficiency, blood deficiency, yin deficiency, deficiency of Yang, different organs deficiency and some other factors),2. "sthenia pathogenic"(Qi stagnation, blood stasis, phlegm, etc.),3. "weakness-sthenia combination pathogenic". The major pathology related internal organs are lung, spleen (stomach), and kidney; especially, kidney is the key cause factor. Kidney imbalances also cause sweet urine, more urine and other symptoms, and kidney disease will directly affect the function of other internal organs, diseases of other internal organs also affect kidneys. Because of the diet and lifestyle changes, modern physicians on diabetes knowledge also are also change, there are somewhat similarities and differences between the clinical symptoms and diabetes. In addition to follow the history of the physicians on diabetes treatment, spleen phlegm, promoting blood circulation and removing blood stasis are the treatment progress of modern diabetes.2. Clinical investigation:Clinical investigation of 607 Cases of diabetes patients in Taiwan area are studied, although more women than men have a slightly higher incidence, there is no obvious statistically significant difference; we also find that most diabetes patients’ age is greater than 51 years old; their bed time is later than 11 pm, and their sleep hours are less than 6 hours, that accounted for about half the number of cases; Families have history of diabetes also accounted for more than 50%; Based on the type of work, unemployed (retirees and housewives) accounts for 284 cases, and in this category,61 years old or above accounts for 236 cases, occupies 83.10%, and office staff stand next; Based on the physical shape characteristics category, skinny, lanky and fertilizer account for 304 cases, obese、skinny-become-obese account for 233 cases and in the second place; Based on type of diabetes in the Western medicine, type 2 diabetes are in the majority; Based on traditional Chinese medicine constitution type, "qi deficiency" accounts most; Based on type of syndrome differentiation of traditional Chinese medicine, "weakness pathogenic" class accounted for 52.22%; Based on the syndrome differentiation of general sthenia syndromes, "sthenia pathogenic" category accounted for 13.01% of the total number of cases.; the syndrome differentiation is a "weakness-sthenia combination" class accounts for 34.77% of the total number of cases. TCM syndrome differentiation of diabetes type in "Qi Yin Deficiency" syndrome type 178 cases accounted for 29.32% are most popular, "spleen deficiency and phlegm" in 68 cases accounted for 11.20% is in the second place.Conclusion:Diabetes of TCM etiology including "internal", "external", and "not internal and external" three factors. Based on the data of the survey, more than half of the patients have family history of diabetes. Based on bed time and sleep hours, for those bed time is later than 11 pm, and their sleep hours are less than 6 hours, that accounts for about half the number of cases; This study also found that heterogeneity of congenital hereditary and the lifestyle of living factors, these factors cause pathogenesis of diabetes, are consistent found with ancient physicians Diabetes scholars. Indulgence in sex, excessive drinking and eating are the main reasons of ancient pathogenic, mostly for the rich people, and the incidence of diabetes of ancient pathogenic is less than modern pathogenic, and the symptoms to "heat pathogenic" and "sthenia pathogenic" account majority. For the modern diabetes, because of the improvement of the living standard and abnormal lifestyle, become common disease in most family, especially happen in elderly people. The society in Taiwan is widespread eating Feigan Atsumi and food supplements, health food diet, and most Taiwanese also sleep late and have sleep problems, furthermore, some food use recycled oil, industrial starch and chemical additives, the long-term effects of the damage internal organs of the body function, health, indirect cause damp pathogenic for diabetes and its complications. So the clinical manifestations of diabetes in Taiwan have more "weakness pathogenic" type and "weakness-sthenia combination pathogenic" type. Clinical in the prevention and treatment of diabetes, should establish the right idea of health, Diabetes prevention should be advocacy; have diabetes disease, make sure to avoid complication occurrence; gain control or cure the diabetes, prevent the disease recurrence. Based on above diabetes prevention concepts, we can have a significant effect in the prevention and treatment of diabetes.
