Rehabilitation Effect Of Yiqi Huoxue Recipe On Diabetic Patients With Lower Extremity Peripheral Arterial Disease

Objective:To investigate the clinical effect of Yiqi Huoxue recipe on diabetic patients with lower extremity peripheral arterial disease by the assessment of clinical symptoms and signs,fasting and 2-hours postprandial glucose,blood rheology,color doppler ultrasound.transcutaneous oxygen tension and walking impairment questionnaire of diabetic patients with lower extremity peripheral arterial disease treated by Yiqi Huoxue recipe buyang huanwu decoction.Method:60 cases of diabetic patients with lower extremity peripheral arterial disease were randomly assigned to treatment group (n=30) and control group (n=30), two groups of patients all received basic medicine to control blood sugar; Treatment group received extra Treatment of Buyanghuanwu soup scientific chinese medicine in the morning and evening.4 weeks as a course of treatment,4 courses were observed. clinical symptoms and signs integral, fasting plasma glucose (FPG),2-hours postprandial glucose (2hPG),whole blood viscosity at low shear rate (nbl). whole blood viscosity at high shear rate (nbh), plasma viscosity (NP), erythrocyte electrophoresis time (ET), hematocrit (HCT), fibrinogen (Fb), platelet aggregation rate (PAR), posterior tibial artery diameter and blood flow, dorsalis pedis artery diameter and blood flow rate, percutaneous oxygen partial pressure (TcPO2), walking impairment questionnaire (WIQ) of two group patients were Evaluated before and after treatment. The effect of clinical efficacy and quality of life in diabetic patients with lower extremity peripheral arterial disease treated by the method of Yiqi Huoxue Tongluo were analyzed in the end.Results:all patients completed the treatment and evaluation according to the test requirements. ①The effect on clinical symptoms and signs integral:after treatment, the total effective rate of treatment group was 83.3%,the total efficiency of control group was 56.7%,the comprehensive clinical effect of treatment group was better than control group (P< 0.05);②The effect on FPG and 2hPG:after treatment, the FPG and 2hPG of treatment group were improved than before (P<0.05), and was better than the control group (P< 0.05), while the control group has no significant difference than before(P> 0.05);③The effect on blood rheology:after treatment, the nbl,nbh,np,ET,HCT,PAR of treatment group were improved than before(P<0.05), and the nbl,nbh,HCT,PAR was better than the control group (P<0.05), while the indicators of control group and Fb of treatment group has no significant differences than before(P>0.05), and there was no significant between two groups in np, ET, Fb (P>0.05);④The effect on posterior tibial artery diameter and blood flow:after treatment, the posterior tibial artery and dorsalis pedis artery vascular diameters of treatment group were expanded wider than before (P< 0.05), blood flow also increased than before (P<0.05), and was better than control group (P< 0.05); while the vascular diameter and blood flow of posterior tibial artery anddorsalis pedis artery in control group has no significant difference than before (P> 0.05); ⑤ The effect on lower extremity microcirculation:after treatment, the TcPO2 of treatment group was significantly improved than before(P<0.05), and was better than the control group (P< 0.05); while the control group has no significant difference than before (P>0.05);⑥The effect on quality of life:after treatment, the walking speed and distance,WIQ comprehensive score of treatment group were significantly improved than before(P<0.05), and was better than the control group (P<0.05);while the control group has no significant difference than before(P>0.05),the stair climbing ability of treatment group has no significant than before and control group (P>0.05).Conclusion:1 Yiqi Huoxue Recipe can effectively improve the clinical symptoms and signs of diabetic patients with lower extremity peripheral arterial disease,and is beneficial for their glycemic control.2Yiqi Huoxue Recipe can achieve the purpose to treat diabetic patients with lower extremity peripheral artery disease by improving their blood rheology indexes,blood flow velocity and microcirculation of lower limb,and expanding their lower limb vascular diameter.3Yiqi Huoxue Recipe can improve the exercise capacity of diabetic patients with lower extremity peripheral artery disease on the basis of the improvement in clinical symptoms and signs and lower limb blood supply and demand,so as to improve their quality of life.
