Purpose:To analysze and classify the indications, clinical presentations, and surgical outcomes of intraocular lens (IOL) explantation or exchange over a5-year period.Method:This retrospective study comprised135eyes of133patients who had IOL explantation between2008and2013. The clinical characteristics, indication, type of removed and replacement IOL, time interval of explantation, effect on vision were recorded and analyzed.Results:The patients were divided into two groups according to replantation or not. IOL dislocation (39%), retinal problems(38%), glaucoma(19%), corneal endothelium decompensation(13%) and endophthalmitis(6%) were the most common indications for remove without exchange, while IOL dislocation (74%), incorrect IOL power (10%) and optic opacification(8%) were the main reasons for exchange. Two extract peaks were observed after IOL implantation. One was less than one year, and the other was from7to12years. Of the IOLs removed,1-piece hydrophobic acrylic accounted most (38%), with polymethylmethacrylate being the major replacement IOL (72%). Over all, the mean logMAR visual acuity improved significantly after IOL exchange (P<0.05).Conclusions:The leading indication for IOL explantation was lens dislocation, followed by Incorrect IOL power, optic opacification and corneal problems,which were all decreasing. IOL exchange was an safe and effective way to manage IOL related complications.
