| Irritable bowel syndrome (IBS) is a kind of typical mental and physical disease, it manifested as the symptoms of abdominal pain with abnormal defecating, increased year by year, at present the treatment methods with western medicine and psychological therapy are in the majority to relieve or eliminate the main symptoms for the purpose, but they are easy to stimulate intestinal mucosa and aggravating intestinal burden, produce big side effect. The cost is great, they are hard to keep the curative effect. Acupuncture treatment method is simple and convenient, it is cheap and has good curative effect. But overall the existing research literature it is found that acupuncture treatment of IBS experimental literature and acupuncture and moxibustion therapy function mechanism of IBS research are little, the research of points specificity mechanism of acupuncture treatment of IBS is blank field.At present the pathogenesis of IBS focus on brain-bowel axis disorder and gut feeling high sensitivity. Therefore, this study is going to explore the mechanism of acupuncture treatment of IBS and specificity of acu-points mechanism through observing the chang of the brain-gut axis. Whether is there the influence on acupuncture treatment for the brain-gut axis disorders in IBS model animals? What is the action segment target? What kind of acupuncture points are specific effects, and what is their mechanism? These problems are important for our study. It has important significance to choose specific acu-points of treating IBS and study their mechanism.They can enrich point specificity theory research content and present solid basis for the clinical treatment.Using8~21days of newborn rats to make IBS model, the model rats are given continuous rectum acetic acid stimulate. Three couples common points at different parts of the body are used in this study to observe their different analgesic effects. They are ST36and SP9on the lower limbs, SJ4and LI4on the upper hand, RN4and ST25on the abdomen. With rats abdominal withdraw reflex (AWR) score as the evaluation index of sensitivity of intestinal pain. With the change of brain-gut peptide content such as NPY, SS, CGRP and CRF at the different targets including the thalamus, spinal cord, plasma, colon parts in brain-gut axis to compare the effect difference of above points for IBS, furthermore to discuss the mechanism of their differences. There are80newborn rats in this study. The rats are randomly divided into normal group, the model group, EA ST36group, EA SP9group, EA SJ4group, EA LI4group, EA RN4group, EA ST25group. The EA are given to EA groups from the6th week, totally seven times of treatment,20minutes every time, the density wave,2/100Hz alternating frequency, intensity0.1-0.3mA, wave wide0.20.6mS.After treatment, the rats are accept the visceral sensitivity evaluation with the rats abdominal retraction reflection (AWR). Then the rats are killed, their thalamus, spinal cord, plasma and colon tissue are taken to be checked their bioactive substance content. Neuropeptide Y (NPY) content in the thalamus, spinal cord and colon tissue of the rats are checked by the radiation immune analysis mothed.Somatostatin (SS) content in the plasma and the colon tissue of the rats are checked by Enzyme immunoassay method. Corticotropin-releasing factor (CRF) content in the hypothalamus of the rats is checked by Enzyme immunoassay method. Calcitonin-gene-related peptide (CGRP) content in the thalamus, spinal cord and colon tissue of rats are checked by RT-PCR method. Using SPSS17.0software, the results of the study are as follows:1Effect of electroacupuncture on different acupoints on visceral sensitivity for IBS ratsElectroacupuncture on different acupoints have different analgesic action for model rats with abdominal pain.Compared with normal group, the capacity threshold of AWR of the rats of model control group on abdominal lift and arches its back lowered significantly, p<0.01.After electro-acupuncture treatment:compared with model control group, the capacity threshold of EA ST36group, EA SJ4group, EA RN4group rats on the abdominal lift increased significantly, p<0.05. EA SP9group, and EA LI4group, EA ST25group rats the capacity threshold does not change, p>0.05. Compared between every two groups, p>0.05.Above results shows the visceral sensitivity of the bowel of the model rats increased, ST36, SJ4, RN4can reduce the experimental intestinal high sensitivity of the rats, ST36, RN4and SL4points which have strong function of calming mind have best effect, while the action of LI4, SP9and ST25points is not obvious.2Effect of electroacupuncture on different acupoints on NPY content in different targets of brain-gut axis for IBS rats2.1Effect of electro-acupuncture on different acupoints on NPY content in thalamus for IBS rats Content changes of NPY in the thalamus tissue of the brain can show the height of anti-stress ability, the rise of the content of NPY shows the body can strongly against the stress stimulator.Compared with normal group, NPY content in thalamus of model rats is reduced, p<0.05.Compared with model group, EA SP9group, EA SL4group, EA LI4group increased obviously, statistically significant, p<0.05; EA ST36group and EA RN4group increased significantly, p<0.01; EA ST25group compared with model group increased not obvious, no statistical significance, p>0.05.Above the results showed that EA treatment can enhance the rat anti-stress ability by improving NPY content in the thalamus. ST36and RN4points which have strong function of calming mind have best effect, while the action of SL4, SP9and LI4points the second, ST25point has no obvious effect of improving anti-stress factors.2.2Effect of electro-acupuncture on different acupoints on NPY content in the spinal cord for IBS ratsCompared with normal group, NPY content in the spinal cord of model group rats no change, p>0.05.Compared with model group, NPY content in the spinal cord tissue of all EA groups change no obviously, p>0.05.Above the results showed that the spinal cord is the middle area of the information collection between the brain information and intestinal sensory information, it is the important link of brain-bowel regulation pathways, but the adjust action of EA treatment is not obvious here.2.3Effect of electro-acupuncture on different acupoints on NPY content in the colon for IBS ratsCompared with normal group, NPY content in the colon tissue of model rats is reduced, p <0.05. Compared with model group, NPY content in the colon tissue of all EA groups rats increased in varying degrees.Among them EA SP9group and EA ST25group increased, p<0.05. EA SL4group, EA RN4group, EA ST36group and EA SL4group compared with model group not increased, p>0.05.Above the results showed that EA treatment can regulate the disfunction of bowel movement by rising NPY content in the colon tissue. ST36, RN4, SL4and LI4points which have strong function of calming mind have no obvious effect, while SP9and LI4points which have action of only local regulation without obvious function of calming mind have best effect. The result shows that SP9point and ST25point can treat IBS by adjusting NPY content in local colon.3Effect of electroacupuncture on different acupoints on SS content in different targets of brain-gut axis for IBS rats3.1Effect of electro-acupuncture on different acupoints on SS content in the plasma for IBS ratsSS is typical of brain-gut peptide, it can participate in visceral sensation conduction, it release to influence the colonic motion through adjusting the numerous hormone.Compared with normal group, plasma SS content of model rats increased significantly, statistically significant, p<0.05.Compared with model group, SS content in plasma of EA ST36group, EA SL4group, EA RN4group decreased obviously, statistically significant, p<0.05; That of EA SP9group, EA LI4group and EA ST25group changed not obviously, no statistical significance, p>0.05.Above the results showed that EA treatment can regulate function of bowel by reducing the SS content in the plasma of IBS rats. ST36point, RN4point and SL4points which have strong function of calming mind have good effect, while the effect of ST25, SP9and LI4points have no obvious effect.3.2Effect of electro-acupuncture on different acupoints on SS content in the colon for IBS ratsSS has an important adjusting function to gut sensation and movement. It is an important brain-gut peptide in the bowel tract.Compared with normal group, the SS content in colon tissue of model rats increased, statistically significant, p<0.05.Compared with model group, EA LI4group decreased obviously, p<0.05; EA ST36group, EA RN4group and EA SL4group decreased, p<0.01.Above the results showed that EA treatment can regulate function of bowel by regulating the SS content in the colon of IBS rats. ST36point, RN4point and SL4points which have strong function of calming mind have best effect, while LI4points the second, ST25, SP9points have no obvious effect.4Effect of electro-acupuncture on different acupoints on CGRP content in different targets of brain-gut axis for IBS rats4.1Effect of electro-acupuncture on different acupoints on CGRP content in the thalamus for IBS rats CGRP can participate in forming pain, it is necessary material to produce allergic and pain sensation.Compared with normal group, CGRP content of model rats in the hypothalamus increased significantly, p<0.05.Compared with normal group, CGRP content in the hypothalamus of EA LI4group, EA RN4group and EA ST25group increased significantly, p<0.05.Compared with model group, CGRP content in the hypothalamus of EA LI4group, EA RN4group continues to rise, p<0.01.Compared with model group, EA ST36group, EA SP9group, and EA SL4group no change, p>0.05.Above the results showed that the pain substance increased after model was made and EA treatment can regulate and control to upload the pain stimulate signal, as well as paticipate in hyperalgesia formation by regulating the nerve center senior CGRP content.The four points include ST36point, SP9point, SL4point and ST25point and the two points include LI4point and RN4point have opposite trend of regulation.4.2Effect of electro-acupuncture on different acupoints on CGRP content in the spinal cord for IBS ratsCGRP and SP coexist in the spinal cord, CGRP may strengthen the transmission of pain signals by SP.Compared with normal group, model rats CGRP content increased significantly in the spinal cord, p=0.01.Compared with normal group, CGRP content rise in the spinal cord of EA ST36group, EA SP9group, EA SL4group, EA LI4group, EA RN4group and EA ST25group increased obviously, p<0.05.Groups compared with model group, CGRP content rise in the spinal cord of EA ST36group, EA SP9group, EA RN4group increased obviously, statistically significant, p<0.05. EA ST36group increased more obviously, statistically significant p<0.01.Above the results showed that pain signal in the spinal cord in model rats transfer strengthen, EA treatment can regulate the Intestinal pain sensitive by regulating CGRP content in the spinal cord of IBS rats. ST36point which has strong function of calming mind have best effect, while RN4point the second, LI4point, ST25point, SP9point have no obvious effect.4.3Effect of electro-acupuncture on different acupoints on CGRP content in the colon tissue for IBS rats CGRP is important brain-gut peptide, in the colon area where it can participate the produce process of pain feeling in intestinal local by influence SP.Compared with normal group, CGRP content in colon tissue of model group increased significantly, p<0.05.Compared with model group, CGRP content in colon tissue of EA ST25group, EA SP9group and EA RN4group increased obviously, statistically significant, p<0.05, among them EA RN4group increased more obviously, p<0.01.Compared with model group, CGRP content in colon tissue of EA ST36group, EA SL4group, EALI4group increased not obviously, p>0.05.Above the results showed that effect is obvious when proximal points or lower limb local points including RN4point, ST25point, SP9point are taked through EA, but far end points or upper limbs points not. That showed local points or lower limbs points which located in the close ganglion section will have best adjust action for brain-gut peptide content of the colon local area, instead, adjust action is poor.5Effect of electro-acupuncture on different acupoints on CRF content in thalamus for IBS ratsCRF is a major media in central nervous system which paticipete in anxiety and other stress response, and may adjust the gastrointestinal tract through HPA axis.Compared with normal group, CRF content in thalamus of model group no change, p>0.05. Compared with model group, CRF content in thalamus of all EA groups have no obvious change, p>0.05. Above the results shows, the adjust effect of EA to CRF are not obvious.conclusionResearch proves that different points of rat models have different effect of intensity to their visceral high sensitivity state. The evaluation for rat behavioral response shows, campared with the model rats, the effect of EA treatment to abdominal raise capacity threshold is most obvious on ST36point, RN4point, SL4point. Campared with the model rats, the effect of EA treatment to back arched capacity threshold is most obvious on ST36point, RN4point, SL4point. The effect of EA treatment on other points is not obvious. The results proved the three points include ST36point, RN4point, SL4point are the best points. Different points adjust the high sensitivity of bowel of model rats through different ways. We speculate that different points adjust the bowel highsensitivity by adjusting imbalance in the body of the endocrine hormone levels on many targets.In view of the three points include ST36point, RN4point, SL4point have the best effect for bowel highsensitivity, we found the change of content in many detecting indexes, after EA treatment on above three points, content of the thalamus NPY, plasma SS and colon tissue SS are all change greatly, it is not obvious between other indexes change and EA effect of points. We speculate that NPY in brain and SS in plasma and local colon may be key regulators in brain-gut axis for the action of acupuncture and moxibustion, thus leading to the difference of acupoint effect. The left three points include SP9point, ST25point and LI4point can only obvious adjust the content of brain NPY or just intestinal SS, it is postulated that is one of the mechanism which the left three points have weak adjustment effects.Why do ST36point, RN4point and SL4point have obvious regulation effect? From the point of view of acupuncture and moxibustion dialectical location, the above three points have the action both of calming the mind and adjusting bowel. So paying attention to acupoint selection with dual function of calming the mind and adjusting bowel in the treatment of IBS clinical process is especially important, it is easy to find these kinds of points have more clearly adjustment action from the brain and bowel two parts and function, they can produce faster and more remarkable regulation effect. The concept of brain-gut axis provides theory basis for further understanding spirit psychological factors on the influence of the gastrointestinal tract pathophysiology, and ascend such disease from gastrointestinal local to the whole body, it provides a new idea to explore the specificity of acu-points in acupuncture treatment for gastrointestinal diseases. |
