Objects:To explore the effects of RKABT on the reproductive endocrine and ovary in PCOS in clinical practice and granulose cells in vitro in experimentsMethods:1.Clinical researchment:Divide 53 cases of PCOS sufferers of kidney deficiency and blood stasis type in syndrome differentiation into observe group treated with RKABF and control group treated with. CC+HCG.The course is 3 menstrual cycles.The period,quantity,color and quality of menstruation,symptoms in TCM,the size of ovary and the number of follicles under Bmode ultrasonic,BBT and female gonadotropin around treatment are observed.2.Experimental researchment:The granulose cells are segregated from the approaching mature follicles of rats.Then divide them into four groups-observe2 group cultured with rats' serum with Restoring kidney and Actirating Blood Formula(RKABF) component,positive control group cultured with pregnant mares serum,blank group cultured with rats' serum without medicine and model group.The proliferation and form of cells,secretion of gonadotropin and the expression of IGF-1 and IGF-1 receptor are observed in four groups.Results:1.Results of clinical researchment:①The rates of ovulation concluded from BBT in both of the observe group and control group have no distinct difference(p>0.05).②The chief symptoms such as soure knees and legs,tiredness and inertia,vertigo and tinnitus and aching of ankles are obviously alleviated in observe group.It has distinct difference in comparison with control group.③The accumulated points around treatment in observe group have significantly decreased.It has significant difference(p<0.01).④The size of ovary and the number of follicles around treatment in both groups have been notable ameliorated(p<0.01).⑤The level of hormones around treatment is obviously decreased in both groups.The decreasing of the level of T and the rate of LH/FSH in control group is striking and of significant difference in comparison with observe1 group(p<0.01或p=0.05).2.Results of experimental researchment:①The proliferation of granulose cells of rats which have been cultured with 10%of serum with.RKABF component is best in four groups.②The effect of serum with RKABF componet on the morphology of granulose cells is superior to the other three groups.③The secretion of E2 is accelerated in observe2 group.It has distinct difference in comparison with blank group(p<0.01).While the secrection of T is inhibited in observe2 group.And it has distinct difference in comparison with the other 3 group(p<0.01).④The expression of IGF-IMRNA in observer2 group and IGF-1 protein are inhibited.It has distinct difference in comparison with blank group and model group.Besides,the expressions of IGF-IRmRNA and IGF-IR protein are promoted in observe2 group and of distinct difference in comparison with other 3 groups.Conclusion:As for accelerating ovulation,RKABT has as the same effect as treatment with CC+HCG.With regard to clinical symptoms such as menoxenia,infertility,enlargement of ovarian polycyst,sour knees and legs, vertigo and tinnitus and aching of ankles,the therapeutic effects of RKABT is superior to treatment with CC+NCG.It's of great importance to improve the sufferer's life on the basis of ameliorating ovarian function.RKABT can significantly decrease the enlargement of ovary and the number of atretic follicles,diminute the volume of ovary,promote ovulation and inhibit the development of atreticle follicles.In addition,RKABT can decrease the high level of LH and T in PCOS, ameliorate the rate of Ln/1=sn,and increase the level of E to regular the axis of reproductive endocrine.The serum with RKABT componet is superior to serum with pregnant mares in improving the growth and the secretion of E2 and decreasing the level of T2 of granulose cell of rats in vitro.Observe2 group with serum with RKABF component and Positive control group with pregnant mares serum are superior to blank group and model group in promoting IGF-I genes,And serum with RKABF can downregular the expression of IGF-I proteins.The expression of IGF-1 protein is highest in model group.It suggests that the effects of the serum with RKABT component on accelerating the proliferation of granulose cells are associated with the increase of the expression of IGF-1 not with IFG-1.
